The Allied Health Services (AHS) team consists of Regulated Healthcare Professional including: Physiotherapy, Occupational Therapy, Speech Language Pathology, Social Work, Registered Dietitians, Respiratory Therapy, as well as unregulated Rehabilitation Assistants and Communicative Disorders Assistant. Each discipline is focused on driving evidence informed practice to promote high quality patient and family centered care.



The team has a professional practice leader (PPL) model for each regulated allied health discipline which promotes and enhances the interprofessional team. Each discipline has their own PPL with dedicated time to enhance their communities of practice, represent their respective disciplines at committees, create policies & procedures and lead corporate level initiatives.


We are currently seeking a

Temporary Full-Time

Social Worker to support our inpatient acute critical care team.


Qualified candidates will have the following requirements:


Education:




Graduate of an accredited School of Social Work with a Master of Social Work (or equivalent)- Required Current registration with the Ontario College of Certified Social Workers/Social Services Workers -Required Evidence of continuing education in related field- Required Sound knowledge of related legislation as applicable to serviced population- Required

Experience:




Recent Social Work experience involving seniors in an acute care emergency medicine and/or community setting - Preferred. A strong theoretical and practical knowledge of social worker role and scope in promoting independence and assistance in acute care/ED setting- Preferred. Strong knowledge of community services, supports and experience related to facilitating family meetings and discharge planning- Required. Experience with quality improvement initiatives or projects an asset.

Ability to function co-operatively as part of an interprofessional assessment team. Possess comprehensive assessment, treatment, and documentation skills Possess excellent interpersonal and communication skills Demonstrate sound clinical judgment, flexibility and initiative Ability to communicate effectively with families, caregivers, and other professionals. Superior organizational and time management skills are essential to management of the caseload.

Other:




Ability to learn about, from, and with other members of the patient care team to foster a strong interprofessional model of care. Demonstrate an ability to appreciate the importance of and be open to new concepts and the ability to trial new approaches. Responsible for acting as a team lead on occasion as the role will rotate within interprofessional team members. * Ability to provide direction and synthesize information from other disciplines to create a comprehensive care plan and present recommendations to the most responsible physician
