Education and Training:
- Successful completion of a University Bachelor degree in Social Work
- Currently registered as a Social Worker with the Ontario College of Social Workers and Social Service Workers
- Membership in the Ontario Association of Social Workers preferred
Experience:
- Minimum of two (2) to three (3) years clinical experience preferably in an acute care setting
Knowledge:
- Must have demonstrated knowledge of the following:
- Consent to treatment act
- Substitute decisions act
- Consent and Capacity Board
- Powers of Attorney
- Knowledge of community resources/supports
- Knowledge of the long term care placement process is an asset
- Demonstrated knowledge and management of ethical issues
Demonstrated Skills/Ability:
- Proven problem solving skills especially in exceptional and unique situations
- Demonstrated proficiency in Microsoft Office Suite software, e.g. Word, Excel and Outlook
- Demonstrated excellent interpersonal and communication skills with the proven ability to communicate effectively with patients, physicians and other health care professionals
- Proven organizational skills and ability to prioritize workload is essential
Personal Suitability:
- Demonstrated ability to work as a member of a multidisciplinary team, as well as being able to work independently in a fast pace environment with challenging clients and families
- Willingness to learn from the professional knowledge of colleagues and other disciplines
- Demonstrated positive work record and good attendance record
