You will work in collaboration with our Social Worker and the Leadership team to advocate and provide support and services to residents and families of Maple Manor Nursing Home.

SKILLS AND KNOWLEDGE REQUIRED
Diploma in Social Service Work
Member in good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW)
Experience in long-term care or related healthcare setting preferred
Specialized knowledge or interest in gerontology and mental health is an asset
Strong interpersonal, communication and advocacy skills
Understanding of the Long-Term Care Homes Act and relevant health legislation
Familiarity with community resources and support networks
Organized, compassionate, and resident-focused with a commitment to service excellence
Experience in using Point Click Care for documentation and assessments
Ability to work independently and as part of a team
Commitment to continuing education in geriatric social work
RESPONSIBILITIES
Help residents adapt to transitions, grief and loss, and other challenges
Working with residents in the evening with anxiety, dementia, depression, isolation and emotional and social challenges
Collaborate with Social Worker and BSO RPN during the week to help support interventions for residents with behaviours and coach other staff members in evening about interventions
Resource for evening/night staff
Provide leadership on conflict prevention/resolution between families and residents
Complete assessments on residents as needed
Develop and implement individualized care plans
Advocate for the rights and needs of residents
Provide counselling and emotional support to residents and families
Assist with end-of-life planning and decision-making
Promote social and recreational activities for seniors in the evening
Documentation for residents completed
Assist residents that need support during social events (i.e. dining, entertainment)
Evaluate the effectiveness of care plans and help adjust as needed
Other duties as assigned by the Administrator

ABOUT MAPLE MANOR
Maple Manor Nursing Home, locally owned and operated, is a long-term care facility located in the downtown core of Tillsonburg Ontario. The home is licensed by the Ministry of Health and Long-Term Care and is Accredited through the Person-Centred Long Term Care Community Standards of CARF Canada. The Home is a 101-bed, smoke free facility offering private, semi-private and ward accommodations. In addition, Maple Manor has four temporary licenses for Interim Long-Term Care Home beds through the Aging at Home program. These beds provide a location for residents who would otherwise be kept in hospital while awaiting placement into a Long-Term Care facility.
