We are seeking a

Social Service Worker

for a permanent full-time rotation.

The Social Service Worker, reporting to the Director of Care, supports the delivery of psychosocial support to our residents, families and staff. The Social Service Worker provides assessment, treatment, and evaluation of individual, interpersonal, and societal problems through the use of social service work knowledge, skills, interventions and strategies, to assist individuals and families, to achieve optimum social functioning.

The Social Service Worker deals with enhancing the resident's well-being and quality of life by promoting and respecting the dignity and individuality of each resident in order to support self-determination, independence and optimal social functioning. The Social Service Worker deals with the resident including loss of function/ability/roles/ lifestyle changes, while supporting significant others in the resident's life and assisting families in advocating needs. The Social Service Worker promotes continuity of care for residents of long term care, through the identification and use of services available in the community. In collaboration with Home and Community Care, the social service worker facilitates the discharge planning process from long-term care to community services if required, thereby promoting the most beneficial placement for the resident. The Social Service Worker will also support families as they deal with transitioning their loved ones into long-term care and staff as they require psychosocial support.

As a member of the interdisciplinary team, the Social Support Worker provides input into initial, annual and other resident assessments, care planning and evaluation to ensure services are planned and updated according to resident needs considering privacy, cultural diversity, intimacy, dignity and quality of life.

Responsibilities and Duties:



Social assessment and counseling Assists and supports new resident/substitute decision maker with admission into the Home. Provides one-to-one counseling where necessary and may refer to external resources for further counseling for the purpose of facilitating the resident's adjustment to the Home, supporting psychosocial well-being, and supporting quality of life dimensions. Provides clinical interventions to residents, families and staff, utilizing appropriate intervention and counseling techniques allowed within scope of practice from governing body. May also develop Support Group services for residents, family and staff. Provide appropriate treatment to assist resident/family adjust to living in the Residence; assist with behavioral/life adjustments. Coordinates and supports discharge planning as required, provides education regarding available resources and arranges for appropriate services, including referrals to community resources and arrangements for appropriate follow-up.
Consultation and collaboration

Provides counseling within scope of practice to resident, family member or staff regarding loss, disability, end of life and adjustment to long term care reality. Supports the resident, their caregivers and staff by providing counselling, group or individual, according to the College of Social Work and Social Service Workers scope of practice Completes social assessment for all new admissions. Participates as a member of the interdisciplinary team, attending and providing social service work input and consultation as needed. Participates at resident and family interdisciplinary care conferences. Prepares and presents educational sessions to residents/families/significant others related to the social aspects of illness and disability. Assists residents and families in administering and accessing power of attorney, public guardian and trustee and other financial resources available. Meets with all new families and residents to orientate them to Maxville Manor and answer any questions. Provides orientation and education to the staff of Maxville Manor in regard to the creation of a therapeutic environment; healthy interpersonal relationships; enhancing staff understanding of the normal aging process and the Resident Bill of Rights; heightening insight into early recognition and intervention of psychosocial problems; and promoting referral of residents to the Social Work Services.
Administrative functions

Participates in, and actively contributes to professional specific as well as program-based quality management, research and education activities. Develops a comprehensive knowledge base regarding community resources (associations, programs and services) and develops positive partnership with them, making referrals as appropriate. Records appropriate information in PointClickCare and maintains written records in accordance with the policies of Maxville Manor and as required by the LTCH Act and professional licensing standards and guidelines. Attends Maxville Manor committees where a social service worker vision would be beneficial. Keeps accurate workload statistics in accordance with as well as those required by the Ministry of Long-Term Care legislation and Maxville Manor. Provides orientation and supervision to social services work students when appropriate.
Other

Performs other related duties allowed under the Ontario College of Social Workers and Social Service Workers as directed by the Director of Care.

Qualifications:



One (1) to two (2) years relevant experience in a health care setting, preferably working in a residential setting such as a long-term care home.

2-year Diploma from a Social Service Worker Program from accredited college institution.

Demonstrate relevant experience with the elderly, dementia, behavioral disorders and palliative care. Member of the Ontario College of Social Workers and Social Service Workers, in good standing Understanding of relevant Regional and Provincial legislation, including LTC Homes Act, Consent to Treatment Act, substitute Decisions Act, MFIPPA, PHIPA and policies and procedures. Current knowledge related to the Consent and Capacity Board and Powers of Attorney. Knowledge of community, resources, programs and services, and issues pertaining to older adults, including knowledge of human behaviour and counselling techniques. Demonstrated skills in client/family assessment, intervention and assistance, counselling, advocacy, case management and service planning, including assisting them in developing a remedial plan of action. Demonstrated organizational ability to handle caseload requirements, compile statistical data and reports for review and evaluation. Experience with documentation in an electronic health record program preferred (e.g. PointClickCare). Ability to work outside of normal business hours on occasion as required. Working knowledge of MS Office, including Word, Excel and Power Point.

Pay: $60,000.00-$65,617.00 per year
