Reports to:

Seniors System Navigator Team Lead

Position Summary:



Provide in-home and community socialization services and connection to older adults within Lethbridge.

Detailed List of Responsibilities:



To perform this job successfully the individual must be able to achieve the responsibilities listed below with excellence, focusing on work quality, timeliness, and precision.

All LSCO employees are expected to perform responsibilities to meet organizational and funding requirements as directed by mandate and policy.

Client Support & Programming:



? Carry a caseload of socially isolated older adults who have been assessed to benefit from in-person contact to increase socialization, self-esteem, and self-worth;

? Provide general information concerning government and non-government programs, benefits and services to seniors including relevant legislation and regulations (Personal Directive Act, Social Housing Act, Protection for Persons in Care Act etc.);

? Refer client back to Seniors System Navigator Team Lead when additional services are needed, such as system navigation or advocacy.

Administration:



? Support quarterly collection of data required by funders;

? Responsible for all data entry, case notes and reports pertaining to position;

? Maintain private and confidential information;

? Identify program improvement opportunities;

? Keep up to date on resources and information pertaining to seniors;

? Other duties as assigned.

Education & Qualifications:



? Bachelor/Diploma in Recreational Therapy, Public Health, or related education;

? Experience working with the senior population and/or relevant experience;

? Ability to maintain boundaries and assess for emerging needs of older adults;

? Able to work independently but collaborate within internal and external multidisciplinary teams;

? A broad knowledge of personal and social issues affecting seniors and caregivers;

? Adequate experience and skill with personal computers and office productivity software;

? Valid driver's license and $2,000,000 liability insurance:

vehicle required;



? Current Criminal Record Check (within 90 days of employment)
