Position

We are looking for a Seniors Community Connector who will work with the day-to-day operations of Parkgate Society\xe2\x80\x99s Social Prescribing program. The connector will work one-on-one with at-risk and isolated seniors to find appropriate programs and services in their areas. This position will work both in-house and in the community. The Seniors Community Connector will contribute to the overall effectiveness and success of our services for seniors, helping them to feel better connected to the community so they can live life better.

Responsibilities

  • Connect with medical and health professionals to receive senior participant referrals
  • Interview referred seniors to determine their needs
  • Attend seniors\xe2\x80\x99 social programs to determine suitability
  • Navigate and refer participants to local services and programs
  • Monitor participants\xe2\x80\x99 program effectiveness and adjust programming schedule as required
  • Record program data and statistical information to maintain and complete reporting requirements
  • Oversee volunteers\xe2\x80\x99 placements, schedules and performance
  • Build and maintain relationships with program participants and address any questions, suggestions, concerns, and complaints when they arise
  • Assist with advertising and promotion of seniors programming at Parkgate Centre
  • Perform other duties and responsibilities that may arise, based on the needs of the program.
Qualifications
  • A background in social services or social programing
  • Experience with seniors\xe2\x80\x99 programs and services in a community-based organization
  • Working knowledge of not-for-profit and community-based organizations
  • Strong interpersonal communication skills
  • Demonstrated teamwork, facilitation, and leadership skills
  • Flexible and adaptable to change
  • Demonstrated ability to work independently as well as part of a team, and
  • Ability to work effectively with program staff, volunteers and not-for-profit or publicly funded groups, agencies and organizations.
