SNAP is an evidence-based premier

children's mental health and crime



prevention program

for the middle years (ages 6-11)

created in Toronto, Ontario

by the Child Development Institute (CDI) in 1985. It was developed in conjunction with the Toronto Police Service to fill an identified gap in services for young children in conflict with the law when Canada raised the age of criminal responsibility from 7 to 12 in 1984.


Today, SNAP is meeting the needs of children and youth with serious disruptive behavior problems and their families. It works with the child, family, school, and community.


The SNAP program is uniquely committed to the equitable treatment of all communities. We create differential support for communities that have less access to mental health services, such as Indigenous and remote communities, providing gender and culturally responsive programming to ensure each child and family receives the effective mental health support they deserve based on their level or risk and need.



To date, SNAP has created two culturally responsive Guides for Indigenous and Black and Afrocentric communities to ensure the program is a right fit for communities interested in implementing it. These are being implemented at the


community level across Canada and around the world.




The SNAP Child Worker is responsible for providing children's mental health

services within the scope of practice appropriate to the role. The work may be

delivered to individuals, groups or families.



DUTIES AND RESPONSIBILITIES



Delivers children's mental health group interventions (e.g., leads or co-leads

SNAP Boys, SNAP Girls, co-leads SNAP Parent) according to current

SNAP program manuals.

Works with SNAP Family Worker who leads process,conducts

screening interviews and completes the client intake process, including

administration of Eco-systemic assessment, child and parent measures

and the collection of all required intake data.

Delivers children's mental health programs and services within the

scope of practice as outlined in the client treatment plan (e.g.,

individual child counselling/skill development, school

support/advocacy, service navigation).

Actively participates in the case management process for clients

including participation in case management activities and attending

case review meetings.

Liaises, consults and co-ordinates with required professional

(internal and external) and community services to ensure effective

clinical treatment.

Ensures that client documentation in SNAPiT is up to date. Supports the signing and obtaining of consent forms. Provide limited follow-up service to discharged clients (e.g., support,

service navigation).

Maintains agency records as required. This may include

assessment reports, intervention plans, closing reports, pre/post

measures, case notes, statistics and other documentation as

required by agency standards and agency client information

system.

Works collaboratively with other professionals and organizations

on cases.

May perform other related duties as assigned.



QUALIFICATIONS



Education and Experience

Completion of a college or university program in social work, child and youth care, counselling or other social science or health-related discipline. Plus, one year work or volunteer experience in a social service environment working with at risk youth and their families in a culturally diverse, low-income community.

OR

3 years' experience volunteering or workingwith at risk youth

and their families

in culturally, racially diverse communities and low-income communities, may replace formal education. Ability to work in a highly distractible environment; sensitivity to the uniqueness and demands of a public service agency. Maintains the confidentiality and security of client-related and agency information. Proficient with MS Office Suite (Word, Excel, Outlook). Proficient with using videoconferencing systems, such as Zoom as well as Microsoft Teams, which is a business communication platform. Possess strong verbal and written communication and influencing skills. Demonstrated ability to probe, analyze, synthesize and problem solve issues. Be a dedicated and flexible team player able to interact with team members and colleagues at all levels of the organization. Demonstrated ability to work in a respectful manner with groups from diverse backgrounds and experiences

Technical / Job Specific Knowledge and Abilities



Knowledge of child and adolescent development, children's mental health issues, behavior modification techniques, family dynamics and crisis intervention Engagement, problem solving, rapport building and conflict resolution skills with hard-to-reach clients Knowledgeable about non-violent crisis intervention techniques and harm reduction approaches Ability to work independently and as part of the team Ability to exercise mature and professional judgment Good problem solving and conflict resolution skills A highly motivated self-starter with the ability to coordinate multiple projects and tasks Knowledge of and adherence to HRM confidentiality requirements Proficiency with Microsoft Office software Flexible, adaptable, and capable of working in a fast paced, professional environment Availability to work evenings and some weekends Strong written and verbal communication skills
Maintain professional, confidential, documentation, in the form of assessments, case notes, discharge summaries and statistical information via online ESAT note program and SNAPit. Process intake and referral forms to be used when referring families to services Complete paperwork while adhering to privacy act requirements Represent HRM in a positive and professional manner

Security Clearance Requirements:

Applicants may be required to complete an employment security screening check.

Please note

- Testing may be conducted as a component of the selection process to assess technical and job specific knowledge. Candidates, who are selected for testing, may be tested in a group setting, scheduled at the employer's discretion.

HOURS OF WORK

SALARY

: Non Union, $30.20 per hour (plus 4% vacation pay)



WORK LOCATION

: St. Andrew's Community Centre, 3380 Barnstead Lane, Halifax. Travel to other recreation centers will occur weekly.
