Sunbeam Developmental Resource Centre - Waterloo Region (SDRC) is the single point of access to developmental services for children in the Region of Waterloo. SDRC provides service coordination and clinical services to children, youth and adults with Intellectual Disabilities and/or an Autism Spectrum Disorder (ASD), living in Waterloo Region. Utilizing a family-centered, collaborative approach to service delivery, the Service Planning Coordinator, serves as the single point of contact for families eligible for coordinated service planning in Waterloo Region. Service Planning Coordinators will support families of children and youth with multiple and/or complex special needs by acting as one identifiable point of contact for the development of a Coordinated Service Plan recognizing all their service needs and building on their child/youth's strengths. Service Planning Coordinators will help families connect with the appropriate services in their service delivery area and monitor how children and youth are progressing through the service plan as they grow. While the focus of the Service Planning Coordinator will be on developing a family-centered coordinated service plan for the services required for the child/youth, the coordinator will also be expected to make appropriate linkages to resources and supports for the parents and/or family as a whole in order to enable them to better support the needs of the child/youth. Working in collaboration with partner organizations in the community, this role navigates system complexity to ensure the needs of the child, youth and family are met. A high degree of creativity, flexibility and innovation will be required.

Role and Responsibilities

Identifying key goals of the child/family Supporting families to access the services they need including (but not limited to): Developmental and diagnostic assessments Rehabilitation services Respite funding and services Recreational programs Parent groups, supports and information sessions Primary care services Mental Health services School based services and supports Initiating and developing a Coordinated Service Plan collaboratively with the family and all relevant service providers. Facilitating the coming together of relevant providers in the children's services, education, and health sectors in each service delivery area. Facilitating access for the family to relevant services in their service delivery area. Collecting information related to the service needs, strengths and priorities of families and their children and youth with multiple and/or complex special needs. Collaborating with local case/service resolution mechanisms when child and family needs are sufficiently complex and exceed the ability of local services. Monitoring, reviewing, and updating the coordinated service plan at regular intervals (a minimum of 6 months) or as needed (i.e. when goals are met, priorities change), in collaboration with family and relevant providers in the children's services, education, and health sectors, as the child/youth develops. Providing families with information regarding available services in the service delivery area, including services delivered in schools, as well as in Section 23 Programs in Care, Treatment, Custody and Corrections and Section 68 School Authorities. Initiating contacts for an integrated transition planning process in collaboration with the young person and their parent/guardian, the school, community agencies, and health care providers when a young person is preparing to transition to adulthood. Participating in all relevant training related to Coordinated Service Planning, locally and provincially. Developing and maintaining strong relationships with partner organizations ensuring sufficient communication and seamless service delivery. Collecting and reporting all required data and outcomes as identified by the Ministry of Children and Youth Services. Service Planning Coordinators will support the family through their service experience, as needed, until the child transitions to adult services. They will link families with the right information and help them understand and manage their short and long-term service goals. They will also maintain partnerships with children's service providers, District School Boards, health care, recreation services and other services providers

Qualifications:

Bachelor of Social Work or equivalent Extensive experience in working with children, youth and families in the social services sector Strong knowledge of the services and supports available to children complex/special needs in Waterloo Region. Experience in mediation Familiarity with and understanding of the role of Developmental Services Ontario Experience with the special challenges presented by Dual Diagnosis is a definite asset Excellent consulting and oral/written communication skills. Valid driver's license and reliable transportation, required. Preference will be given to those who can communicate in French
