The Service Coordinator will provide direct support to clients. They will perform administrative tasks related to the operation of programs and will assist/liaison with staff for the implementation and maintenance of all services.

ACCOUNTABILITIES



Client and Family Centric Care Delivery




Plans, organizes, and schedules the service delivery for clients including such services as homemaking, personal care, medication assistance, grocery shopping, transportation, and security checks.


Completes client intake for the In-home Support department, receives referrals through HPG as needed including pre-screener assessments.


Establishes a working rapport with clients that supports their dignity, self-direction and that facilitates their activities of daily living (ADLs) and medical needs.


Supports clients with medication reminders related to taking their medications at the scheduled time(s) and in accordance with their physician's instructions, as required.


Complete Security checks and procedures related to safety.

Planning and Organizing




Schedules staff to accommodate new client referrals or to change from existing schedules, and to back-fill staff absences.


Reconcile and complete payroll information for PSWs.


Inputs and maintains case notes and regularly reviews clients' Care Plans, complete documentation.


Inputs client information and service data into client data base, hard copy files and keeps additional manual records as required.


Creates, maintains, and updates client files.

Customer Services




Establishes a working rapport with clients that supports their dignity, self-direction and that facilitates their activities of daily living (ADL, IADLS) and medical needs


Refers clients, caregivers, and professionals to appropriate program & services at Lumacare and/or other community resources as required


Manage inquiries by providing information to callers and direct phone calls when necessary


Support department coverage including Home at last, Meals on Wheels reception, as needed.

Collaboration




Collaborates with Team, Service Coordinators, Care Navigators and Supervisor to ensure scheduling and planning of personal support service are accurate and appropriate


Assists clients/caregivers in connecting clients to internal and external resources, as appropriate.


Documents and informs Supervisor of any incidents or emergency situations


Promotes a collaborative work environment among staff and supports team building activities


Participate in Team Meetings


Collaborates with our Home and Community Care, Toronto Community Housing Corporation and other external partners as necessary.

Administration and other tasks




Participates in internal and external committees/meetings, training sessions


Supports staff in the delivery of safe and effective services which meet client care needs, agency contractual and quality improvement meeting organization standards. Promotes Occupational Health and Safety practices.


Maintains knowledge, observes, practices and enforces appropriate procedures in regards to safety, quality and environmental standards.


Provides after hours on-call duties as required. Travels using own vehicle.


Performs other duties as required

SCHEDULE



Wednesday to Friday:

7:00 AM - 3:00 PM or 8:00 AM - 4:00 PM

Saturday & Sunday:

3:00 PM - 11:00 PM

ROLE REQUIREMENTS




Personal Support Worker Certificate, Administrative certificates as considered as asset.


Experience in staff scheduling and in payroll required.


Experience working with vulnerable older persons and older adults with mental health issues, addictions, and other special needs.


Demonstrates excellent knowledge and skill in customer services.


Strong organizational and time management skills.


Excellent computer skills, including proficiency in word processing, spreadsheet, and data base applications (Word, Excel, Outlook, Microsoft Suite, Teams).


Valid Ontario Driver's License and access to a vehicle.


Immunization for tuberculosis, chickenpox, mumps, and hepatitis B.


Current First Aid & CPR Certification.


Current Vulnerable Sector Police check required completed within last 12 months is required.


This position requires you to be fully vaccinated against COVID-19 and you must provide Lumacare with proof of vaccination prior to employment.
