Position Description
Reporting to the Supervisor of Service for Partners and Organization, the Service Coordinator - Intake, Services for Partners and Organizations (SPO) will provide intake and service coordination to service providers and partner agencies for their clients directly or in service of their clients through the lifespan. This role involves comprehensive intake and screening of clients, coordination of services within Surrey Place, and active engagement with partners and stakeholders.
Main Responsibilities
Conduct intake interviews with service providers, partners, organizations, and staff to assess suitability for services and events.
Prepare and maintain intake documentation and enter data into the Centre's Management Information System (MIS).
Coordinate with internal teams to deliver solutions, such as building quotes or facilitating service access.
Engage in community outreach to foster a collaborative approach to service delivery.
Develop and maintain strategic partnerships with community organizations, healthcare providers, and service agencies to enhance coordinated care for individuals with developmental disabilities.
Facilitate collaboration between internal teams and external partners to support integrated service planning and delivery.
Identify and pursue new partnership opportunities that align with organizational goals and client needs.
Serve as the primary liaison for partner agencies, ensuring timely communication, issue resolution, and relationship management.
Monitor partnership performance and gather feedback to improve service coordination and outcomes.
Generate leads by identifying potential service opportunities and initiating outreach to new partners.

Qualifications
Undergraduate degree in social or health sciences or equivalent
1 years of related experience working with children and youth with developmental disabilities and their families, with a focus on community engagement
Experience working with children and youth with developmental disabilities and their families
Knowledge of developmental services and educational, community, and health care service systems for children with developmental disabilities
Excellent communication and interpersonal skills
Experience on an interdisciplinary team and with multicultural environments is an asset
The successful candidate must provide an acceptable Vulnerable Sector Screening
Proficient with Microsoft Office
Fluency in French is considered an asset
