Position Overview
The Seniors Hub Support Worker is an integral member of the Wellbrook Place Integrated Seniors Hub team, dedicated to enhancing the quality of life for seniors in the community.
This multifaceted role involves providing comprehensive support across various aspects of seniors' daily lives--ensuring their physical, emotional, and social well-being. The Seniors Hub Support Worker is committed to delivering holistic, person-centered care in a supportive and inclusive environment that respects each individual's needs and preferences.

Key Responsibilities
Assist seniors with activities of daily living (ADLs), including bathing, dressing, grooming, and toileting.
Provide mobility support, including transferring, positioning, and ambulation assistance.
Plan, organize, and facilitate recreational and social activities tailored to the interests and abilities of seniors.
Encourage participation in programs that promote physical, mental, and emotional well-being.
Foster a supportive and inclusive environment that combats social isolation and promotes social interaction.
Document participation and progress in recreational activities; provide feedback to the multidisciplinary team.
Assist with feeding when necessary, ensuring dignity and respect at all times.
Maintain cleanliness and organization of dining and activity areas, following health and safety regulations.
Maintain accurate and timely documentation of care provided, observations, and interactions.
Participate in care planning and care conferences with families and interdisciplinary teams.
Conduct assessments and make referrals as appropriate.
Perform other related duties as assigned.
Qualifications
Education
High School Diploma or equivalent (required).
Understanding of person-centered care principles and the unique needs of seniors.
Familiarity with age-related health conditions and their management.
Knowledge of recreational programming and activity facilitation for seniors.
Awareness of dietary needs and preferences, including special diets and food allergies.
