Job Description

ConnectWell Community Health is seeking a Senior Manager, Primary Care member to join our team. This is a full-time, 35 hrs/week position. This position provides a comprehensive benefit package, including Healthcare of Ontario Pension Plan (HOOPP).

Position Summary:

The Senior Manager, Primary Care will provide strategic and operational leadership for the delivery of interprofessional primary care services across all the ConnectWell Community Health's sites.

Qualifications:

Master's degree in Nursing, Public Health, Health Administration, or related clinical or health leadership field (or equivalent combination of education and experience). Current professional registration with a relevant Ontario regulatory body (e.g., RN, NP, SW, PT) is considered an asset Minimum 8 years of progressive leadership experience in a community-based, primary care or interdisciplinary health environment. Minimum 3 years of proven experience leading clinical teams and managing interprofessional models of care. Experience overseeing complex clinical workflows and performance targets.

Reporting Relationship:

The successful candidate will report to the Director, Community Health Centres.
