OVERVIEW




This position is a contract position until October 31, 2026 with possible extension to oversee and carry out implementation of the East Toronto Seamless Care Optimizing Patient Experience (SCOPE) Program, administered by the East Toronto Family Practice Network (EasT-FPN) and Michael Garron Hospital (MGH).


SCOPE is a virtual, interdisciplinary program connecting primary care providers with hospital services and community health partners to improve patient care and access.


The EasT-FPN is the Primary Care Network within the East Toronto Ontario Health Team (East Toronto Health Partners).The mission of the EasT-FPN is to represent the voices of primary care providers (family physicians and nurse practitioners), in East Toronto, through a collaborative OHT decision-making structure. The EasT-FPN has evolved rapidly from its initial inception in June 2019, as a response to an identified need for a voice from the family practice community, and its subsequent partnership with ETHP.


This position ranges between: $61,074 - $71,852


RESPONSIBILITIES




Works collaboratively with EasT-FPN SCOPE Clinical Lead, on the implementation of SCOPE, including developing operational work plans and reports, supporting the overall program. Engages and works collaboratively with the SCOPE Program Operations team, the EasT-FPN Director of Operations, the EasT-FPN and key community partners such as the ETHP Backbone Team, on the design and implementation of projects related to supporting SCOPE-registered primary care providers and meeting SCOPE's objectives and KPIs, including increasing East Toronto primary care providers' access to a comprehensive range of interdisciplinary supports. Supports the development and execution of SCOPE Program operational work plans, the design and sharing of KPI reports, preparation of stakeholder engagement meetings, regular data collection and validation, and preparation of presentations and scorecards. Coordinates and facilitates SCOPE Program team meetings, working groups meetings, quarterly meetings, advisory committee meetings, by developing meeting agendas, materials and presentations, and the follow up on action items, issues and responses, as required. Prepares timely, accurate and comprehensive briefing notes, meeting minutes, project updates, promotional materials, internal/external correspondences and other executive documentation related to program and project planning, project coordination, management, and project reporting and tracking. Attends, takes meeting minutes and follows up on action items for biweekly SCOPE Primary Care Working Group meetings with other SCOPE sites, and presents with the East Toronto SCOPE team as scheduled. Attends, takes meeting minutes and follows up on action items for regional SCOPE Steering Committee meetings - quarterly, and plans meetings when East Toronto SCOPE site is hosting. Attends and supports SCOPE Physician Advisory Group meetings. Prepares and facilitates SCOPE Program provider and patient experience surveys, analyzes results and prepares reports and follow up plans. Co-supports the development and implementation of the East Toronto Primary Care Census, as well as the follow up from the Census, as it relates to SCOPE. Supports communication, marketing strategies (i.e. SCOPE e-blasts and EasT-FPN membership newsletter), CMEs and events, related to supporting SCOPE-registered primary care providers, East Toronto primary care providers and EasT-FPN members' access to a comprehensive range of interdisciplinary supports. Organizes and conducts primary care engagement activities, together with the EasT-FPN and the SCOPE Program team, including preparing materials, coordinating speakers, taking meeting minutes, and following up on action items. Manages the expansion of the SCOPE program across East Toronto by coordinating the development of new SCOPE pathways, enhancing existing pathways, and documenting all processes and steps to facilitate the pathways. Works with the EasT-FPN Director of Operations & Finance and SCOPE Clinical Lead, to prepare program budgets and track expenditures. Organizes and supports regular meetings and updates with East Toronto primary care providers, SCOPE-registered primary care providers, and specialists to discuss progress, challenges, and successes in care coordination and access to care. Identifies and addresses barriers to effective collaboration and patient referrals between East Toronto primary care providers, SCOPE-registered primary care providers, EasT-FPN members, and specialists, working with teams to resolve issues.

EDUCATION




Bachelor's degree in health or business administration required

QUALIFICATIONS




3-5 years of Project Coordinator experience Demonstrated understanding of local health issues, priorities and needs while recognizing the broader trends in health care policy and system development required Sound written and verbal communications skills to write presentations, speeches, business notes, etc. is required Ability to plan and implement engagement meetings and resolve issues/concerns Demonstrated experience and ability to build positive working relationships with partners, influence decision making and adoption of change initiatives Facilitation skills to develop and conduct meetings with multiple stakeholders and service Ability to develop work plans and lead concurrent projects, to work collaboratively with partners and other team members and to manage deadlines and competing priorities. Ability to develop and monitor budgets and agreements/contracts Experience in data analytics and information synthesis Knowledge of health care reporting requirements Good work and attendance record is required.
