Job Overview:

We are looking for compassionate and skilled Registered Practical Nurses to provide high-quality care to our clients. If you have a passion for helping others and making a difference in people's lives, this role is for you.

Responsibilities:

Develop and modify care plans in response to residents' changing status Liaise with other healthcare personnel, agencies, and families Maintain good relationships with medical/support staff, residents, families, and the general public Supervise unit activities and take charge as required Monitor performance and complete evaluations for Personal Support Workers (PSWs) Direct, assign, and evaluate the work of PSWs Utilize leadership skills to identify and solve problems on the unit Ensure care delivery is consistent with the resident Bill of Rights Understand and work towards improving quality indicators used by the facility Manage nursing supplies and equipment efficiently Conduct unit rounds at the beginning and end of shifts Administer medications and treatments in compliance with pharmacy provider policies, facility policies, and College of Nurses standards Maintain accurate and complete records of nursing observations and care Assess changes in health status and report to Substitute Decision Makers (SDMs) and the interdisciplinary team

Qualifications:

Valid Registered Practical Nurse license in Ontario Strong clinical skills and knowledge of long-term care and/or home care environments Excellent communication and interpersonal skills Ability to work independently and as part of a team Leadership and problem-solving skills Commitment to ongoing professional development
