The Transitional Care Unit is a 40 bed unit that cares for patients who have a firm discharge destination but face a temporary obstacle, such as awaiting home and community care services or slow stream rehabilitation and convalescent care. Focused, short term supports will enable these patients to be more quickly and safely discharged. Patients are transferred to this unit once a discharge plan is in place. The healthcare team consists of physicians, registered nurses, registered practical nurses, patient care assistants, dietitians, pharmacists, physiotherapists, occupational therapists, a discharge planner, social worker, a respiratory therapist, a speech language pathologist, a chaplain, unit clerks, and volunteers. Each member has a specific role to play to ensure optimal health and seamless transitions. They strive to provide exceptional care to each patient and family. As the Registered Practical Nurse (RPN) you are a professional caregiver who is responsible and accountable for providing quality patient care within a delivery model in accordance with the organizational philosophy, policies and standards of care of the Royal Victoria Regional Health Centre, the College of Nurses (CNO), and all relevant legislation.
