Overview

We are seeking a dedicated and compassionate Registered Practical Nurse (RPN) to join our healthcare team. The ideal candidate will have experience in various healthcare settings, including acute care and assisted living, and possess strong skills in medication administration and case management. This role is essential in providing high-quality patient care, ensuring the well-being of our residents, and supporting their health needs.

Responsibilities

Deliver direct patient care, including medication administration and monitoring vital signs. Develop and implement individualized care plans in collaboration with the healthcare team. Conduct assessments to evaluate patients' physical and emotional health. Provide support for patients with dementia, pediatrics, and other specialized needs. Maintain accurate medical records using electronic health record systems such as Epic. Communicate effectively with patients, families, and interdisciplinary team members regarding care plans and progress. Assist with laboratory tests and procedures as required. Ensure compliance with medical terminology standards and best practices in patient care.

Requirements

Valid RPN license in good standing. Experience in acute care, assisted living, or similar healthcare environments is preferred. Proficiency in medication administration and case management. Knowledge of laboratory procedures and medical terminology. Experience working with pediatric patients or those requiring dementia care is a plus. Strong interpersonal skills with the ability to work collaboratively within a team. Ability to manage multiple tasks effectively while maintaining attention to detail.
