What will you do?

The Care Connector works within a prescribed mandate, helps patients navigate through a complex health care system by connecting people to an appropriate health care provider while also managing waitlists.

This is a temporary full time (1.0) Registered Practical Nurse position.

What must you have?

Registered Practical Nurse, in good standing with the College of Nurses of Ontario. Good knowledge of primary care services and other community resources as well as the roles of health care professionals. Experience working with various health disciplines as well as the public, interagency contact. Understanding of the long-term care and health care delivery system in the province of Ontario. In-depth knowledge of tools, systems and databases used in patient service delivery and management. Excellent interpersonal, communication, assessment, problem-solving, and decision-making skills Effective time management, prioritization and organizational skills, with the ability to work independently and co-operatively in a busy multidisciplinary environment Established ability to accurately complete required documentation, reports and forms A valid driver's license and access to a reliable vehicle Proficiency in a Windows environment We have a mandatory COVID-19 vaccination policy. As a condition of employment, all employees are required to submit proof of COVID-19 vaccination status prior to start date
