I feel welcome and at home.\xe2\x80\x9d .Job Position: Registered Nurses (RNs) and Registered Practical Nurses (RPNs)Type: Visiting NurseProgram: MGH2HOMEStatus: Full Time, Guaranteed Hours (37.5 Hours / week)Shift: 08:00 - 16:00 (8 Hours / shift)Region: Toronto Central, East YorkWhat will be your role?This is a unique nursing position supporting East Toronto Health Partners\xe2\x80\x99 (ETHP) strategy to improve client transitions from hospital-to-home through the newly established MGH2Home program. This short-term transitional program (up to 90 days) involves linking together Michael Garron Hospital (MGH), Spectrum Health Care, VHA Home HealthCare and WoodGreen to support earlier discharges of adults with higher needs to reintegrate into their home and community. This multi-disciplinary team will work as \xe2\x80\x9cone team\xe2\x80\x9d, along with the client and designated family, to address their complex health and social needs in the development of a single \xe2\x80\x9cone team\xe2\x80\x9d care plan.Grounded in our commitment to Person and Family Centered Care the key focus on the RN/RPN role is to provide and coordinate home health nursing services to diverse group of individuals, families, and groups according to the College of Nurses of Ontario Practice Standards, the policies and procedures of VHA, and the Standards of Practice in Community Health Nursing. This position involves providing direct nursing care within the patients\xe2\x80\x99 home environment, and assessing clients who are eligible for this program in person at MGH.There is a $2,000 Sign-On Bonus for full-time nurses only.Required Qualifications:

  • Must be registered with The College of Nurses of Ontario with good standing.
  • Must have a current BCLS certificate (at Health Care Provider level).
  • Undergraduate Degree/Diploma in Nursing/Science in Nursing and current certificate of competency (licensure) from the College of Nurses of Ontario required.
  • Competency in IV Infusion required.
  • Minimum 2 years of experience for RN/RPN positions preferred.
  • Good communication and documentation skills.
  • Knowledge of community nursing an asset.
  • Ability to integrate and apply critical thinking skills to client care.
  • Access to a car and willingness to drive to multiple patient homes to deliver client-centered care.
