This short-term transitional program (up to 90 days) involves linking together Michael Garron Hospital (MGH), Spectrum Health Care, VHA Home HealthCare and WoodGreen to support earlier discharges of adults with higher needs to reintegrate into their home and community. This multi-disciplinary team will work as \xe2\x80\x9cone team\xe2\x80\x9d, along with the client and designated family, to address their complex health and social needs in the development of a single \xe2\x80\x9cone team\xe2\x80\x9d care plan.Grounded in our commitment to Person and Family Centered Care the key focus on the RN/RPN role is to provide and coordinate home health nursing services to diverse group of individuals, families, and groups according to the College of Nurses of Ontario Practice Standards, the policies and procedures of VHA, and the Standards of Practice in Community Health Nursing.
