Through the Health Care Provider Contract, Calian employs health care professionals across Canada who are integral to supporting the health and wellbeing of our military members.

About the role:

Calian is currently seeking a Community Health Nurse to add to our dedicated team of civilian professionals working at CFB Halifax. Community Health Nurses (CHNs) are responsible for seven major functions within a Canadian Forces clinic: disease and injury prevention, health protection, health surveillance, population health assessment, health promotion, emergency preparedness and response and infection control. CHNs work in the community and primary care setting and act as a member of the CF Primary Health care team. Their work includes individual or group appointments, public health campaigns, and individual or group health education sessions within or outside the CF health services facility.

The Community Health Nurse will be responsible for completing the following duties:

Manages and participates in immunization and travel medicine instructions.

Manages and participates in desensitization clinics.

Performs screening for tuberculosis and/or other illnesses.

Promotes health improvement initiatives by planning, coordinating, and facilitating health promotion clinics

Assess individual needs for and provides education and counseling related to the prevention of health disorders and the maintenance of quality of life when health is impaired.

Attends clinics that support and guide CF members affected by similar health conditions, documents and makes recommendations for outside referrals.

Participates in the management of the outcome data on population health and assists in the extrapolation of health trends pertaining to individuals.

Promotes health improvement initiatives by planning, coordinating, and facilitating health promotion clinics.

The qualifications:

Licensed with the Nova Scotia College of Nursing (NSCN)

Baccalaureate Degree from a recognized nursing program

Copy of current BCLS certification

3 full time equivalent years of clinical experience within the last 5 years in nursing practice:

applying the principles of immunization and communicable diseases management including immunization schedules, vaccine storage and handling, and infection prevention and control;

delivering patient education such as Chronic Disease Self-Management programs; and

working in Public or Community Health - Communicable Disease Control Program, Travel Health, Primary Care, or Occupational Health environment.
