Purpose

Reporting to the Resident Assessment Instrument (RAI) Coordinator, Restorative Care staff is responsible for the development and implementation of plans of care that focus on optimizing resident independence in activities of daily living.

Restorative Care staff is required to perform their duties in a manner consistent with the core values of the home in a manner that supports the "Colour It" resident led philosophy of care.

LTCHA Regulation:

Section 57

a. Restorative care approaches are integrated into the care that is provided to all residents; and

b. The restorative care approaches are coordinated to ensure that each resident is able to maintain or improve his or her functional and cognitive capacities in all aspects of daily living, to the extent of his or her abilities.

Responsibilities

General

Provides for restorative care in accordance with established policy, physician orders, the resident plan of care within their scope of practice. In consultation with the Interdisciplinary Care Team, develops and implements restorative programs including Functional Mobility Individual and Group Exercise Strength and Endurance Training Range of Motion Speech Therapy Sensory Stimulation Dining Maintains accurate and complete records of resident participation and restorative care delivered Maintains working knowledge of all equipment used in the delivery of nursing and restorative care Seeks direction from the RPN and/or RN when resident need exceeds experience or scope of practice Observes, reports, records resident conditions, and follows care plans as directed by the RN and/or RPN Provides education in consultation with the Therapists through workshops, in-services, and information sessions for residents, staff, family members and volunteers Leads the Lift and Transfer team, provides resident assessments, staff training, in-services, maintains slings, falls prevention equipment and assistive devices. Completes accurate documentation for each resident participating in restorative care services by monthly evaluations and through RAI-MDS 2.0 admission, quarterly, and significant change in condition of the resident. All other duties as assigned

Care Coordination

Coordinates visits for residents/power or attorneys with external care providers and vendors for assistive devises and equipment needs. Provides verbal and written communication of restorative care program goals to the care team for the purpose of implementing programs into the daily routines of identified residents. With each admission and readmission reviews the status of the resident Ambulation and transfer ability Fall risk Continence Quarterly in conjunction with Care Team Participates in conferences with the interdisciplinary team providing care to the residents Reviews residents for potential participation in a restorative care program Reviews and evaluates restorative care goals for residents who are involved in restorative care Assists in the completion of the RAI-MDS 2.0 quarterly reviews Quality Management Ensures completion of monthly fall and skin/wound reports and analysis of monthly audits and indicators. Contributes to the development of recommendations for corrective action to reach corporate mean for clinical indicators. Participate in team meetings including Skin and Wound, Pain Management and Falls Prevention, Responsive Behaviours.

Occupation Health and Safety

Advises supervisor or designate immediately of all staff incidents that may result in an Occupational Accident Claim, any concerns with the physical plant, incidents or injuries and completes required reports. Follows all health and safety policies and procedures. Works safely to reduce the risk of injury to self, co-workers, and residents. Wears personal protective equipment as designated. Maintains a clean, safe environment for residents utilizing infection control principles.

Family & Resident Relations

Contributes to positive customer service with residents and families in day to day work. Deals tactfully and courteously with residents, resident's family, visitors, and staff in a positive manner.

Ministry of Long-Term Care Compliance

Contributes to good working relationships with relevant persons within the Ministry of Long-Term Care (MLTC). Knows MLTC legislation, regulation, and process. Contributes to the implementation of processes and systems to ensure compliance to all standards on an ongoing basis.

Qualifications

P.S.W. Certificate or equivalent. Completion of a recognized post-secondary diploma at the community college level in a health related field with a restorative care focus such as: Restorative Care Program from the Centre for Activity and Aging, Dementia Studies Program, Physiotherapy Assistant Program, Kinesiology, Excellent organizational skills, attention to detail, verbal, and written communication skills. Ability to follow direction and work in a team environment. Understanding of concepts of commonly used computer applications such as Microsoft Word, Excel, and Resident Electronic Records.

Working Conditions

Modern facility, temperature controlled, smoke free environment. Hours of work include scheduled shifts and call-ins, and may include day, evening, night shifts, weekdays, and weekends. There is a requirement to work statutory holidays. Work is in a home-like medical care area and requires interaction with residents and their families. The workplace is fast paced, with frequent disruptions and distractions, while meeting preset deadlines. Must be flexible to changing needs of the residents, staff, and operational issues, as they are presented.

Knowledge and Skill

Personal Support Workers must have completed a Personal Support Worker program approved under the Fixing Long-Term Care Act, 2021 and Ontario Regulations 246/22 or have qualifications as identified under Ontario Regulations 246/22 52 (1), (2), (3). Experience in a long-term care setting is preferable. Proficiency in interpersonal communication skills, both written and verbal. Basic computer skills. An ability to take direction as well as understanding policies and procedures is required. A caring and empathetic attitude is crucial, as well as working pro-actively as a member of the multi-disciplinary team. Ability to work in a team environment.

Impact of Error

Errors on statistical information and/or reporting of information may result in incorrect information that could adversely affect staff and/or management decisions.

Errors may lead to ineffective performance and may result in injury to self, others, and residents. Furthermore, errors may result in a loss of public confidence, credibility and create a liability to the corporation.
