At Grandview Lodge the Responsive Behaviours Registered Practical Nurse (RPN) is responsible for the physical, social, and recreational well-being of the Residents. The Responsive Behaviours RPN will focus on mitigating and managing responsive behaviours and improving the quality of life for older adults with mental health, substance use, dementia, delirium, and other neurological disorders exhibiting responsive behaviours.

Education
College Diploma in Practical Nursing
Current License to practice from the College of Nurses of Ontario or eligible to apply for a temporary license from the College of Nurses of Ontario
Current Standard First Aid and BLS (CPR)

Experience
Six (6) months to one (1) year related clinical work experience (preferably in a Long-Term Care setting) or other relevant work nursing experience in management of responsive behaviours.

Knowledge/Skills
Ability to recognize any change in condition in the Resident's usual patterns
Demonstrated knowledge and skill working with individuals with various types of dementia, delirium, mental health, addictions, or other neurological conditions
Demonstrated resident-focused care supported by evidence-based clinical best practices
Ability to provide guidance and direction to a peer group
Ability to work independently and as a team leader
Good understanding of non-pharmacological and therapeutic behavioural approaches
Good understanding of pharmacological agents, and their interactions, that are used to manage conditions that have behavioural components along the disease continuum
Demonstrated enthusiasm, flexibility and commitment to change management
Strong communication skills (verbal, listening, written, and documentation)
Ability to think critically and problem solve
Ability to follow direction from the Charge Nurse, Leadership, and Physician/NP
Interpersonal skills to interact with residents, families, co-workers, and the general public
Technology Aptitude:
+ Demonstrated basic computer skills to perform basic tasks on various technological devices and programs (PointClickCare (PCC)) while adhering to cybersecurity and an awareness of best practices.

Responsibilities
The incumbent is responsible for, but not limited to:

Follow the GVL philosophy of delivering care to Residents by utilizing Dementiability and GPA principles.
Ensure Residents' Bill of Rights are upheld.
Work within policies and procedures determined by Haldimand County, GVL and current legislation.

People
Co-chair the Responsive Behaviours Committee in the Home.
Communicate with residents, visitors, and other personnel with a customer service focus.
Participate in the admission process and family conferences and maintain constant communication with Residents/Power of Attorney (POA)/Substitute Decision Maker (SDM) to ensure changes in treatments, medications, or health status are addressed and communicated in a timely fashion.
Provide work direction, work supervision, work assignments, instruction, and education to the interdisciplinary team.
Continually evaluate and assess staff performance.
Undertake continual collaboration and communication with the Director of Nursing, Associate Director of Nursing, and Charge Nurses in the Home.

Functional
Resident Care
+ Coordinates the Responsive Behaviours interdisciplinary team meetings and rounds working collaboratively with PSWs, nurses, social work, pharmacy, physicians, and external partners.
+ Participates in committees and other teams as required.
+ Contributes to the completion of policy reviews and annual program evaluations.
+ Sets goals, creates action plans and ensures follow up is completed in relation to the Responsive Behaviours Program.
+ Assist the residents with activities of daily living according to their personalized plan of care.
+ Administer medications and treatments as prescribed by the Physician/NP. Recordkeeping and Reports
+ Completes all required reports and statistics as required for the Ministry, QIP, etc.
+ Creates, analyses, and presents detailed reports on quality indicators.
+ Oversees the completion of all documentation related to behaviours, DOS monitoring, assessments.
+ Respects and preserves the privacy, confidentiality and security of health care information at all times.
+ Monitor quality indicators and outcome scores to inform program planning and evaluation.
+ Understands how the interRAI assessment integrates with existing clinical tools and provides leadership in streamlining these processes.
+ Adheres to responsibilities and requirements for reporting adverse resident occurrences subject to regulatory and legislative direction. Health & Safety
+ Report any unusual or unacceptable behaviour of staff/visitors/Residents to Director of Nursing or designate.
+ Maintain any equipment used in accordance with established policies and procedures.
+ Adheres to infection prevention and control practices as outlined in the policy and procedure manual.
+ Ensure the Resident's comfort and safety through compliance with current health and safety regulations and Ministry of Long Term Care standards.
+ Report any and all abuse immediately to the Charge Nurse in the Home, Director of Nursing, and Administrator.
+ Complete accident/incident report forms, as well as Critical Incidents.
