Assist in reviewing OHAH Applications to determine if appropriate for admission to the facility when responsive behaviours are present in the application.
RAIMDS and Behavioural assessments for responsive behaviours and all other required documentation.
Consult with Psychogeriatric Resource Consultation (PRC) and Geriatric Mental Health Outreach Teams (e.g. GMOHT, Baycrest, BSOT teams).
Facilitate, coordinate and follow up to weekly interdisciplinary unit behaviour rounds including updating of care plans and 'At-a-Glance' sheets following each meeting.
Educates staff on Best Practice regarding responsive behaviours; collects data related to responsive behaviours. Prepares monthly statistics to present to PAC, inter-professional rounds and all other required meetings, including external reports to BSO office,
Have knowledge preparing and submitting high intensity reports.
Performs other duties as assigned by the Director of Care and CEO.
Flexible hours, depending on the needs of the residents.
Qualifications
:
Minimum 3 - 5 years' experience within a long term care facility
A Registered Practical Nurse with a current certificate of competence with the College of Nurses of Ontario
Computer proficiency, computer literate in Microsoft Word and Excel
Experience with Point Click Care and Point of Care
P.I.E.C.E.S and GPA Trained
Attendance at Montessori Methods of Dementia
Post RPN education in gerontology, either as a certificate or diplomas preferred
Experience dealing with residents with responsive behaviours and mental health conditions
Possession of strong leadership, communication and diplomatic skills
Evidence of good attendance
