We are seeking a dedicated and skilled health care professional to join our healthcare team. In this role, you will be responsible for assessing, treating, and caring for patients with respiratory and cardiopulmonary disorders, with a focus on sleep disordered breathing and/or home oxygen. You will work closely with physicians and other healthcare professionals to provide high-quality patient care in a home care setting. Your expertise in home care will be an asset, in delivering effective treatment plans and ensuring patient comfort. We are open to training the right individual, if your experience hasn't yet involved home care.
Duties
Conduct assessments of patients' overall health with a focus on respiratory health.
Initiate oxygen therapy in the home.
Initiate CPAP therapy to ensure effective therapy and maximize comfort.
Utilize various medical equipment.
Monitor patient progress and adjust treatment plans as necessary based on clinical evaluations.
Educate patients and their families on respiratory care.
Collaborate with interdisciplinary teams to develop comprehensive care plans for patients.
Maintain accurate documentation of patient assessments, treatments, and outcomes in compliance with protocols.
Qualifications
Valid license from CAHPPEI, CLPNPEI, CRNMPEI
Clean criminal record check
Professional Liability Insurance
Experience in a home care setting preferred.
Proficiency in medical terminology.
Strong communication skills to effectively interact with patients, families, and healthcare team members.
Ability to triage tasks for effective work flow and patient care.
Driver's license and reliable vehicle
