Your Opportunity:

Children\'s Home Care is a multi-disciplinary team with RN, SW, RT, PT, OT, SLP, and Rehab assistants who collaborate and deliver services to children from 0 - 18 years old with a wide variety of medical conditions, including children with complex health needs. As a Respiratory Therapist (RT) for Children\'s Home Care, you will act as both a Case Manager and a direct care provider, utilizing your strong skills in family-centered practice, coaching, assessing, organizing, and communication. The service locations can be at a clinic, clients\' homes, and/or community settings. The case management duties include ongoing assessment and identification of the overall unmet needs of the child/family. In collaboration with the family and other partners the case manager develops a care plan, based on those unmet needs, using the appropriate internal and external resources. Case Managers work within a Competency Framework that includes personal and professional accountability, systems thinking, creative thinking, communicating, and building relationships. Principles of adult learning must be incorporated while educating parents and community partners on a variety of health based issues to enable the client to participate to the best of their ability at home and in their community.

Description:

As a Respiratory Therapist, you will assess patients, develop and implement respiratory therapy care plans, and monitor their progress. This role requires you to collaborate with various clinical teams and groups to enhance clinical outcomes as well as educate patients and their families regarding diseases, medications, equipment and treatments. You will be responsible for providing training and assisting health care providers in the use and care of various respiratory diagnostic and therapeutic equipment and interventions. Required Qualifications:

Active or eligible for registration with the College and Association of Respiratory Therapists of Alberta (CARTA). Additional Required Qualifications:

Graduation from an accredited school of Respiratory Therapy. Recent pediatric experience with airway management and respiratory needs. Basic Cardiac Life Support- Healthcare Provider (BCLS-HCP) required. Active registration with the College & Association of Respiratory Therapists of Alberta (CARTA) and to maintain registration throughout the duration of employment. The successful candidate must be able to work in different settings and environments (home environments and weather conditions), as well as be able to lift and carrying medical bags, which weigh approximately 25-30 lbs. Required to bend, pull, lift, and perform variable movements and position changes from sitting on floor doing activities with children, families, and colleagues to working at desk/computer for extended periods of time. Preferred Qualifications:

2 years current Respiratory Therapy practice as a Home Living case manager is preferred. Computer knowledge/skills including Meditech, Connect Care, and Outlook. Strong interpersonal skills are assets. Knowledge of the principles of adult learning is an asset. McMaster Case Management training and understanding of the AHS Continuing Care Case Management Competency Framework. Knowledge of pediatric community resources and programs.
