Position Summary:



As a Resource Pharmacist you will be reporting to the Clinical Manager of Pharmacy Services and you will be an integral member of the Pharmacy leadership team accountable for the development of pharmacists at all Trillium Health Partners sites. You will collaborate with the pharmacy leadership team and work with staff to determine the clinical, technical, teaching and research related needs of staff to facilitate and implement processes to ensure they are met. You will also contribute to peer, pharmacy undergraduate, interdisciplinary and pharmacy residency teaching/training and be an active participant in relevant inter-professional hospital committees.


You will serve as the Resource Pharmacist Lead for pharmacy-related Accreditation Canada standards. In this role, you will coordinate and support the alignment of pharmacy practices with accreditation requirements.


As the Ambulatory Care Resource Pharmacist Lead, you will be responsible for standardizing clinical practices in ambulatory care settings and identifying opportunities for optimization within the health information system.


Occasional travel between hospital sites will be required.



Key Qualifications:





Baccalaureate Degree in Pharmacy or Pharm D. Licensed in Ontario to practice pharmacy (Part A of Register) Completion of hospital pharmacy residency is preferred Minimum 5 years of recent experience in hospital pharmacy practice Experience being a preceptor for students or equivalent training and experience Completion of leadership training and leadership experience is an asset Active membership in the Canadian Society of Hospital Pharmacists (CSHP) is an asset Experience in project management is an asset Demonstrated effective change management skills Strong leadership, interpersonal, problem solving, organizational and communication skills

Responsibilities include but are not limited to

:


Ambulatory care clinical practice and Accreditation Canada Activities

Monitors Accreditation Canada standards and educates leadership and staff on changes. Establishes mechanisms to support compliance with pharmacy related standards including audits and feedback Responsible for implementing and maintaining Medication Reconciliation related Accreditation standards in Ambulatory care settings across THP Promotes, advances, and integrates clinical best practices, evidence-based practice, and quality improvement within the ambulatory care setting for pharmacists Identify opportunities for optimization within the health information system for the ambulatory care setting Coordinates staff orientation to new positions, and/or new practices and procedures. Promotes and facilitates professional development and ongoing learning and education within the pharmacy department providing opportunity to all staff to develop leadership and clinical skills Act as a mentor and resource for staff Active involvement in teaching pharmacy, medical and nursing staff and/or other health care professionals and students Promote Pharmacy Services by participating on relevant hospital committees and professional committees (i.e. PSNs, hospital subcommittees, Pharmacy and Therapeutics Committee, etc). Collaborate with physicians, nurses and allied health Ability to manage and lead large projects and manage change

Administrative and Human Resource Management

Assist in the scheduling of staff to ensure coverage of clinical patient care activities and drug distribution activities at the respective site Assist the Clinical Manager of Pharmacy in the process of performance appraisals for all pharmacists Conduct regular pharmacist team meetings Assist Clinical Manager of Pharmacy in the planning, organizing and directing of pharmacy policies and procedures in accordance with hospital initiatives, professional practice standards and provincial and federal laws Works in conjunction with other Resource Pharmacists and the Clinical Manager cooperatively to prioritize, coordinate and integrate workloads

Clinical and Distributive Services

Review and participate in enhancing comprehensive pharmacy clinical programs Collaborate with Medication Management Pharmacist to review distribution processes training and development Provision of pharmaceutical care to patients, distribution activities, drug order review and drug use evaluations Will be expected to support other duties as assigned including the ability to potentially work at all Trillium Health Partners sites
