This Job is suitable for a Registered Nurse, with at least three years of experience working in a Long-Term Care setting as a supervisor, with strong interpersonal, supervisory and managerial skills, or an LPN, with at least 6 years of experience working in a Long-Term Care setting as a supervisor, with strong interpersonal, supervisory and managerial skills.Key Responsibilities: Provide assistance and support to the clinical team to ensure safe and quality care provision to residents.
Monitor accuracy of RAI-MDS data and proactively engage nursing team to assess residents in a timely manner.
Assist nursing staff in the completion of RAI-MDS coding/RAP\xe2\x80\x99s/CAP\xe2\x80\x99s/Care Plan reviews as per RAI-MDS schedule.
Assist DOC with the evaluation of RAI-MDS data
Participate in quality improvement initiatives relating to PCC & MDS
Report and late/missed RAI-MDS assessments to DOC
Submit RAI-MDS assessments to VCH and CIHI as per schedule, correct rejected files and re-submit files as needed.
Perform monthly audits as needed.
Respond in a timely manner to concerns from residents, their families and staff
Motivate staff and promote and maintain a high standard of PCC & RAI-MDS data submission.
Respond to and manage significant changes in residents\' conditions and attend to medical emergencies.
Monitor care in accordance with the established standards, as per best practice guidelines.
Other duties as assigned.
Required Qualifications: Registered Nurse or Licensed Practical Nurse in good standing with BCCNM or eligible for registration with BCCNM
Comprehensive knowledge of how person and family centered care apply to long- term care concepts and philosophies
Knowledge of RAI-MDS 2.0 and completed coding training course by VCH
Computer skills in Microsoft Office/Outlook/Excel, including Point Click Care, preferred
Experience or willingness to learn to effectively work with seniors, their families and health care sides in Long Term Care
Education and experience in gerontology, rehabilitation and long term care an asset
Must possess strong work ethic and ability to be flexible and work on all floors
Must possess strong written and oral communication skills, critical thinking, coordination, time management and resident and family focus skills
Must demonstrate sound clinical knowledge and ability to work with and provide support to other nurses and health care aides
Aptitude for detail-oriented work and strong problem solving abilities
Current Basic Cardiac Life Support and First Aid certification required
