Job Summary:
We are seeking a compassionate and experienced Social Care Team Lead to oversee and coordinate social care services for our clients. The ideal candidate will have a strong background in caregiving, home care, and senior care, with a focus on providing exceptional support to individuals in need.

Duties:
- Lead a team of social care professionals to ensure the delivery of high-quality care services
- Coordinate and oversee daily operations of social care programs
- Provide direct care and support to clients in areas such as cleaning, dementia care, medication administration, meal preparation, etc.
- Develop individualized care plans based on clients' needs and preferences
- Monitor and evaluate the effectiveness of care plans and make adjustments as needed
- Communicate effectively with clients, families, and healthcare professionals to ensure continuity of care
- Maintain accurate documentation of client progress and services provided

Requirements:
- Proven experience in caregiving, home care, senior care, or related fields
- Knowledge of best practices in nursing home or assisted living environments
- Strong leadership skills with the ability to motivate and guide a team
- Excellent communication and interpersonal abilities
- Ability to work collaboratively with multidisciplinary teams
- Certification or training in relevant healthcare fields is a plus
