The Resident Care Coordinator reports directly to the site General Manager and provides oversight for the care of our residents. .
This is a full-time position, 8-hour days Monday - Friday. The team lead position is compensated at an hourly wage determined by the site General Manager.
The following outlines the specific areas of responsibility and oversite for the team lead:
A. Resident Care
Dispense medications, as required/scheduled.
Make medical appointments for residents and book transportation.
Review team lead communication book and receive handover reports.
Monitor changes in residents' condition and behavior and advise manager of changes.
Update resident care plans on a 3-month schedule while working with Personal Support Workers to maintain same.
Support all visiting primary care providers by providing requested resident information and receive any orders/prescriptions.
New resident orientation - preparing resident charts for admission, assisting manager in location tours, settling new residents in room and making them familiar with routines.
Maintain contact with resident NOK and provide family updates as needed.
Assist new residents with integration, introducing them to other residents and inviting to participate in activities.
Maintain a caring and interested attitude when dealing with residents and families.
Communicate with manager on any resident concerns brought forward concerning residents and/or families.
B. Staff Supervision
Monitor staff for completion of daily duties.
Complete staff audits on a regular basis and forward findings to the manager.
Offer direct feedback and coaching as required
Prepare and distribute shift assignments to Personal Support Workers for each shift
C. Security Duties
Participate/lead emergency drills as required.
Ensure that all staff are familiar with and review the Emergency Plan regularly
D. Administration Duties
Leading meetings when needed.
Answering phone and receiving messages.
Following up on messages in a timely and effective manner.
Completing month end documents.
Assisting General Manager as needed.
