Our Harbour Resident Care Coordinator



Job Types:

Full-time (35 hours / week) and part-time positions available



Salary:

From $28.30 per hour depending on qualifications and experience.

Benefits:

Paid statutory holidays and vacation, reimbursement of km for work (excluding arrival and departure), staff cell phone provided.

Schedule:

Monday to Friday - occasional evenings, weekends and available to be on-call after hours and during weekends.

Our Harbour/Le Havre

is a bilingual, small non-profit organization, funded by donations and grants, providing a long-term, supported housing program for adults living with mental illness. We believe that everyone living with mental illness can be part of a community and live a meaningful life. Our residents live in shared apartments situated in Ville Lemoyne, Greenfield Park and St. Lambert. Our Harbour is the only such program across the Monteregie for Anglophones.

Our Harbour is a demanding, fast-paced environment. The incumbent will report to the Director General and work closely within the staff team and with volunteers. Staff, volunteers and often families work together to ensure that residents receive the best possible care and support.

The Resident Care Coordinator position

involves day-to-day interactions with residents in a supportive capacity, which includes but is not limited to:

? Weekly apartment meetings

? One-on-one interventions that are resident goal-directed

? Advocating/representing residents' needs to members of their medical care team

? Referring residents as appropriate to/for resources

? Monitoring residents' compliance with medication

? Problem-solving on behalf/with/for the residents

? Teaching /implementing life skills (i.e. budgeting skills) to maintain their overall autonomy, health, and well-being.

? Networking/communicating with community partners

Qualifications & Requirements



? Possess a DEC in Social Service Technology or Special Education Techniques, Bachelor of Social Work or Psychology, or a related field.

? Possess a minimum of 2 years' experience working in mental health/illness in an adult population.

? Possess a professional, caring, mature, and responsible work ethic.

? Be able to take initiative and be of an open mindset with a roll up your sleeves, can do attitude.

? Be flexible and able to adapt to changing needs and situations.

? Be able to intervene and manage mental health crisis situations.

? Be able to work as part of a team to support residents to live autonomously.

? Be able to work respectfully and collaboratively with the residents' medical team, families, staff and volunteers who interact with and support individual residents or who provide specific assistance in matters ranging from apartment maintenance to engagement in activities.

? Must have own car and be able to travel to St. Lambert, QC reliably

? Must be punctual.

? Preference for candidates with training in CPR, AED, and First-Aid as well as Crisis Intervention Training for persons working in mental health. Duties

? Conduct evaluations, participate in interviews, and assess the appropriateness of potential residents: team goal 100% occupancy rate.

? Be or become familiar with and understand how referral resources, community agencies, and health services function in Quebec.

? Accompany residents to medical appointments, being present and advocating for their needs as needed.

? Conduct weekly apartment meetings and follow-ups:

a) Review how residents are maintaining their autonomy, including:

i. Monitoring the dynamics of the roommates' relationships and providing guidance when interpersonal conflicts arise,

ii. Ensuring that the distribution of house chores is fair and being taken care of by each resident, and demonstrating, if needed, how to do a task,

iii. Addressing any issues or concerns regarding the functioning of the apartment (i.e. an appliance in need of repair), following up on cleanliness of apartment as well as ensuring that there are groceries in the fridge and that food is properly stored if the resident is not eating regularly, referring residents to community resources, such as food banks, as necessary.

iv. Being available to be on-call for emergencies after hours and during weekends.

b) Use professional judgement to flag behaviours indicating a change in mental health status, explore and discuss with resident(s) their medication (verify if the resident has been taking their medication as prescribed) and refer to the medical team to assess, advocate for additional support, such as Suivi intensif variable (SIV) or Suivi intensif dans le milieu (SIM) services, as may be needed based on functional observations.

c) Manage mental health crises in collaboration with the Resident Care team and Director General and contact the crisis center or 911 when necessary.

d) Create community service plans integrating staff, volunteer and community resources to support individual needs.

? Monitor the work of stagiaires, students or interns who join the staff on temporary assignments and provide them with feedback.

? Liaise with community partners and volunteers to provide integrated programs.

? Maintain complete, accurate records and reports.

? Provide backup support during vacation or absences.

? Undertake administrative assignments as requested (i.e., answering the telephone and taking messages, preparing monthly activities calendar, preparing weekly resident update summary, filing and documentation/progress notes, etc.).

? Assist in maintaining the order and cleanliness of the office and activity spaces (i.e., tidying up after activities and events, putting away dishes, emptying the trash, etc.).

Please note this is not an exhaustive list of duties. Additional responsibilities may be assigned based upon the experience, skills and qualifications of the candidate, including, for example:

? Coordinating specific activities, e.g., creative / arts activities, zoo therapy

? Supervising stagiaires or interns

? If requested, representing the resident care team at Board meetings

? Representing Our Harbour on relevant Tables or community network organizations

? Collaborating with community partners to provide education and awareness about our mission and about mental illness

? Other specific tasks assigned by the DG.

Le havre / Our Harbour

est un petit organisme bilingue a but non lucratif finance par des dons et des subventions. Il offre un programme de logement supervise a long terme pour des adultes vivant avec un trouble de sante mentale. Nous croyons que toute personne vivant avec une maladie mentale peut faire partie d'une communaute et mener une vie significative. Nos residentes vivent dans des appartements partages situes a Ville Lemoyne, Greenfield Park et Saint-Lambert. Le havre est le seul programme de ce type en Monteregie pour les anglophones.

Le havre est un milieu de travail exigeant et dynamique. La personne retenue relevera de la directrice generale et travaillera en etroite collaboration avec l'equipe et les benevoles. Le personnel, les benevoles et souvent les familles travaillent ensemble pour s'assurer que les residentes recoivent les meilleurs soins et soutiens possibles.

Le poste de coordonnateurtrice des soins comprend des interactions quotidiennes de soutien avec les residentes, notamment :



Reunions hebdomadaires dans les appartements Interventions individuelles axees sur les objectifs des residentes Defense et representation des besoins des residentes aupres de leur equipe medicale Referencement des residentes vers des ressources appropriees Suivi de l'observance de la medication Resolution de problemes au nom / avec / pour les residentes Enseignement et mise en oeuvre des competences de vie (ex. : budget) pour maintenir leur autonomie, sante et bien-etre Etablissement de liens et communication avec les partenaires communautaires

Qualifications et exigences



DEC en techniques de travail social, techniques d'education specialisee, ou baccalaureat en travail social, psychologie ou domaine connexe Minimum de 2 ans d'experience en sante mentale aupres d'une clientele adulte Ethique professionnelle, bienveillante, mature et responsable Capacite a faire preuve d'initiative et ouverture d'esprit; attitude proactive et engagee Souplesse et capacite d'adaptation aux besoins et situations changeantes Capacite a intervenir en situation de crise en sante mentale Capacite a travailler en equipe pour soutenir l'autonomie des residentes Capacite a collaborer de facon respectueuse avec l'equipe medicale, les familles, le personnel et les benevoles qui interagissent avec les residentes ou les soutiennent dans divers aspects (entretien, activites, etc.) Posseder une voiture et pouvoir se deplacer de facon fiable a Saint-Lambert, QC Ponctualite requise Une formation en RCR, DEA, premiers soins ainsi qu'en intervention de crise en sante mentale est un atout important

Responsabilites



Evaluer les candidates potentielles et participer aux entrevues afin d'assurer un taux d'occupation de 100 % Connaitre ou apprendre le fonctionnement des ressources de reference, organismes communautaires et services de sante au Quebec Accompagner les residentes a leurs rendez-vous medicaux, defendre leurs besoins au besoin Animer des reunions hebdomadaires dans les appartements et effectuer les suivis, notamment : a) Evaluer l'autonomie des residentes :
i. Observer les dynamiques entre colocataires et intervenir en cas de conflit

ii. Verifier la repartition equitable des taches menageres et demontrer au besoin

iii. Resoudre les problemes lies au fonctionnement de l'appartement (ex. : appareil defectueux), assurer la proprete, verifier la presence de nourriture adequate, referer a des ressources comme les banques alimentaires si necessaire

iv. Etre de garde pour les urgences en dehors des heures regulieres et les fins de semaine

b) Utiliser son jugement professionnel pour reperer des signes de deterioration de la sante mentale, discuter de la medication avec les residentes (verifier la prise conforme), referer a l'equipe medicale, recommander des services comme le Suivi intensif variable (SIV) ou dans le milieu (SIM) selon les observations

c) Gerer les crises de sante mentale avec l'equipe des soins et la directrice generale, contacter le centre de crise ou le 911 si necessaire

d) Elaborer des plans de services communautaires en integrant le personnel, les benevoles et les ressources externes pour repondre aux besoins individuels

Superviser le travail des stagiaires, etudiantes ou benevoles et leur fournir une retroaction Collaborer avec les partenaires communautaires et benevoles pour offrir des programmes integres Tenir a jour des dossiers et des rapports complets et exacts Fournir du soutien lors des absences ou conges des collegues Effectuer diverses taches administratives (ex. : repondre au telephone, messages, calendrier d'activites, resume hebdomadaire des residentes, classement, prises de notes, etc.) Participer au maintien de la proprete des espaces de travail et d'activites (ex. : rangement apres les activites, vaisselle, poubelles, etc.)
