Job Title:

Relative Services

This position reports to:

the Program Director

1. Job Overview:



a) To understand and respond to relative (client) needs while they are at the Centre

b) To liaise with internal and external parties to address relative (client) needs

c) To follow-up with past TTLL clients

d) Related office and clerical duties

2. Key Responsibilities:



a) Actively participate as a member of the intake and discharge teams

b) Consult with Management as necessary regarding special circumstances

c) Maintain relative (client) medical binders and computer files according to procedures

o Ensure all information in medical binder is complete and up to date

o Ensure computer documents are up to date and printed out

d) Pharmaceutical intake and processing according to procedures

o Input existing and new medications in computer and print forms for medical binder

o Assist Counsellor with medication reconciliation

o Liaise with Pharmacy regarding relative(client) medications

o Monitor medications and ensure relatives (client) have enough to last their entire program

e) Arrange relative (client) medical appointments according to procedures

o Booking and follow-up for phone, online, and in-person appointments

o Support relative (client) during their phone or online appointments that take place Monday - Friday between 8:30 am - 4:30 pm.

o Arrange weekend appointments when required

o Provide transportation to appointments during the week using TTLL vehicle

f) Assist relative (client) as required to obtain documents such as ID, SIN, etc.

g) Assist relative(client) or referral workers with return travel arrangements

h) Follow-up with relative (client) after they leave program

i) File all relative (client) medication documents into relative (client) binder

j) Perform clerical duties; maintain accurate files and record keeping systems

3. Duties Include (but are not limited to):



a) Intake & discharge, medical appointments, reconcile medications, travel arrangements.

b) Access and maintain relative (client) medical binders and related computer files

c) Relate positively to relative (client) and encourage them in their program

d) Follow all TTLL Policies and Procedures

e) Attend and participate in weekly case conferences

f) Participate in training, staff meetings, and Circle

4. Additional Responsibilities:



a) Emphasize the importance of Indigenous culture in care and healing journey of relative(client)

b) Work cooperatively with other members of the Program team

c) Submit reports as required or directed

d) Other tasks as directed

5. Minimum Training/Experience Requirements:



a) Completion of High School plus completion of computer and business classes.

b) At least 2 years' experience in a responsible position requiring administrative skills, computer skills, and confidentiality.

6. Desired Additional Training/Experience Qualifications:



a) Adept familiarity with Indigenous culture

b) Excellent interpersonal and communication skills

c) Respectful, helpful, and reassuring attitude

d) Minimum of 2 years abstinence from alcohol and mind-altering drugs; Role model and maintain an abstinent lifestyle.

e) Knowledge of addictions, healing journey, and basic counselling skills in trauma and addiction.
