Job Description

PURPOSE OF JOB:



To provide rehabilitation support to resident's in collaboration with a team of health care professionals.

To encourage residents to participate in rehabilitation programs and treatments to maximize their independence and dignity.

Conduct all actions according to the mission of the Dr. V.A. Snow Centre.

QUALIFICATIONS:



An approved one-to-two-year rehabilitation program OR LPN Rehabilitation program OR be willing to enroll in LPN Rehabilitation program as condition of hire. Applicant to incur cost of the course.

All The Right Moves trainer certification or be willing to complete the Train the Trainer course offered through the New Brunswick Continuing Care Safety Association as condition of hire.

Experience using Momentum Healthware computer program is an asset.

Ability to work with minimal supervision and be flexible in setting priorities.

Commitment to personal, continuous education and training.

The ability to work independently and collaboratively within a multidisciplinary team environment.

Excellent attendance.

A positive attitude and strong work ethic is required.

DUTIES AND RESPONSIBILITIES:



Assists in the development of resident treatment plans to meet the resident's needs such as positioning, mobilizing, feeding, skin integrity and hearing.

Implements and participates in personalized rehabilitation programs in collaboration with health care professionals, to enhance and maintain resident's autonomy.

Initiates all Extra Mural & Metro Health Referrals and obtains family consent.

Works closely with Extra Mural Health professionals to monitor the effectiveness of prescribed equipment.

Arranges all orthopedic and compression sock referrals and appointments.

Communicates and provides guidance to staff pertaining to the rehabilitation programs.

Review resident charts and progress notes to keep up to date on resident's conditions.

Modifies and adapts individual treatment plans in accordance with resident's status as established by health care professionals.

Communicates progress with resident's, families, and staff regarding rehabilitation programs.

Inform family members of rehabilitation care to encourage family and resident involvement.

Perform active or passive range of motion and special physiotherapy when prescribed.

Perform biweekly hearing aid maintenance and checks.

Assist in falls risk assessment as per policy and procedure protocol.

Assist in Adaptive Clothing Assessments, consents etc.

Assess and identify rehabilitation need of residents.

Discuss any concerns about the rehabilitation programs and/or resident progress with the rehabilitation professional.

Maintain good relationships with the residents, families, the public and other employees.

Follow All the Right Moves Policy and Procedures concerning transfers and mobilization of the resident.

Responsible for scheduling and providing All the Right Moves training to all nursing employees in collaboration with the other ATRM trainers.

Complete residents ATRM pictograms on admission and with change in condition.

Initiate a regular daily walk routine for those resident's requiring assistance.

Be a member of wound care committee and attend monthly meetings.

Remain up to date and collaborate with RN &/or LPN with those residents in wound care management program. Assist with completion of weekly wound assessments.

Clean and check all resident wheelchairs once a month as per schedule.

Complete a bed rail/entrapment risk assessment and the 7 zones of bed entrapments measurements when changing mattresses and when adding any additional supports to resident beds. Document all assessments in momentum.

ADMINISTRATIVE DUTIES:



Maintain a complete inventory and catalogue or resident's personal rehabilitation equipment as well as in house equipment.

Communicates with suppliers to obtain quotes regarding rehabilitation equipment.

Order equipment and supplies in collaboration with management.

Participates in meetings such as multidisciplinary team, committee meetings and quality program by providing information and ensuring follow up recommendations.

Maintain detailed progress notes in Momentum for each resident referred.

Complete weekly resident care plan reviews and ensure all required rehabilitation information is part of the resident care plan.

Complete rehabilitation section on resident annual review prior to schedule Annual review meeting.

Complete rehabilitation section on LTCF as per direction of RAI coordinator.

Provide educational in services to staff regarding rehabilitation care.

Organize and keep track of in-home equipment using Momentum Healthware.

TASKS TO SUPPORT REHABILITATION SERVICES:



Obtain signed extra-mural consent form for each referral.

Inform extra-mural professionals of new consultation requests made by the physician and/or Registered Nurse.

Attend multidisciplinary and family meetings.

Regularly check orthotics, bed and wheelchair positioning of identified residents.

Work in collaboration with the maintenance department to ensure all equipment used for treatment or programs is operational and regularly maintained for safety.

HEALTH AND SAFETY:



Observe and practice safe work habits.

Ensure a safe environment.

Report any safety hazards immediately to your supervisor and take steps to remove then if appropriate.

Take part in safety programs provided (Emergency Preparedness, WHMIS, Infection Control etc.).

Use equipment as directed in a safe manner.

Participate in All the Right Moves exercises before beginning work.
