JOB SUMMARY:



The Rehabilitation Assistant is an integral member of the Menno Place team and is required to have a commitment to the vision, mission and values of Menno Place. In performing duties, the Rehab Assistant will follow Menno Place policies and procedures ensuring the provision of Resident and Family Centred Care (RFCC) while safeguarding staff and residents.

Under the direction of a therapist such as a Physiotherapist and/or Occupational Therapist, the Rehabilitation Assistant implements rehabilitative treatment plans and programs, and assists the therapist while reporting to the Recreation Manager.

This position requires teamwork, communication and cooperation. It is the expectation that the individual in this position contributes to a positive, safe working environment, takes instruction well and cooperates with their supervisor and co-workers. This person manages their time well and participates in continuous quality improvement of services provided.

DUTIES & FUNCTIONS:



Carries out designated components of a rehabilitative treatment plans and/or program, such as activities of daily living, mobility, strengthening and balance exercises, recreation, and /or communication therapy. Observes and reports, to the therapist, cognitive and functional changes in resident ability or behaviour that may warrant a progression or regression of the treatment documentation in the health record as directed by Therapist. As part of a multidisciplinary team and in consultation with the therapist, provides input into the planning, implementation, modification, and evaluation of the rehabilitative treatment plan/program. Transfers and positions patients in accordance with established transfer procedures, utilizing mechanical aids such as wheelchairs and lifts to ensure maximum comfort. Provides functional skills training to residents and families in activities of daily living, mobility, and communication as directed by therapist; demonstrates the use of assistive devises. Assists with power and manual wheelchair mobility training. Performs other related duties as assigned.

QUALIFICATIONS



Education, Training and Experience



Completion of a Rehabilitation Assistant Program or an equivalent combination of education, training and experience. Experience in older adult care environments, is an asset.

Skills and Abilities



Ability to communicate effectively in English both verbally and in writing. Ability to deal with others effectively. Physical ability to carry out the duties of the position Ability to organize work. Ability to operate related equipment.

Health & Safety



Is aware of their rights and responsibilities and follow all health and safety policies and procedures. Works safely to reduce the risk of injury to self, co-workers, and residents. Is alert to and promptly reports all actual or potentially hazardous situations to immediate supervisor. Does not operate or use faulty equipment. Promptly reports personal injury to supervisor and seeks first aid as needed. Participates in fire safety demonstrations and fire drills, and knows the facility fire and disaster plan.

Resident Safety



Follows instructions for safety as stated on the care plan Activities of Daily Living and Care Standards Manual; knowing and implementing emergency procedures as required; preventing infection; practising personal care; promoting a safe environment such as safety of equipment and safety of work areas.
