The Opportunity

Reports to the Managers, Rehabilitation Services. The Rehabilitation Coordinator provides clinical leadership in collaboration with Rehabilitation Managers and Medical Director of Rehabilitation. They provide mentorship to an evolving Rehabilitation Services Portfolio to ensure the integration of evidence-informed pediatric rehabilitation care into practice. The coordinator develops pediatric rehabilitation guidelines and processes to support continuous quality improvement. The Rehabilitation Coordinator leads orientation for new staff and preceptorship for all staff and liaises with rehab professionals in the rest of the province to identify pediatric rehabilitation practice gaps and assist in meeting them and building capacity.
Responsibilities include, but are not limited to:
Program Development, Implementation and Evaluation:
Engages and supports program planning, implementation and evaluation of the Rehabilitation Service.
Works collaboratively with the Manager(s) and key stakeholders to implement the strategic directions for the Rehabilitation Service (patient and family goal setting, caseload guidelines, outcome measures).
Seeks information re best practices, and clinical guidelines and facilitates translation of knowledge into practice to support interprofessional rehabilitation practice.
Leads the development of policies and/or standards and guidelines to support Rehabilitation Services.
Leads the development of processes to support translation of knowledge into care for Rehabilitation Services.
Liaises with Rehab colleagues in the rest of the province to identify pediatric rehabilitation practice gaps and to communicate pediatric rehabilitation clinical guidelines as they are developed.
Contributes to the accreditation process for the portfolio.
Mentoring and Professional Development:
Mentors Rehabilitation Team members by leading, supporting, teaching, role modelling, and coaching in a respectful and effective manner.
Supports the manager(s) with education initiatives either portfolio generated or roll-out of centre-wide education initiatives.
Supports the manager(s) with the implementation of development plans for staff.
Organizes and coordinates the rehabilitation orientation for new staff; organizes and monitors preceptoring of new staff from a rehabilitation/program-based care perspective.
Organizational:
Liaises with physicians, Manager and team member(s) to resolve clinical practice issues and improve operations.
Collaborates with team members, Professional Practice Leaders, Managers, Directors, and Quality Resources & Decision Support Services to implement and enhance continuous quality improvement, safety and risk management activities.
Collects and analyses quality improvement data as it relates to the implementation and maintenance of strategic directions.
Participates in the Canadian Network of Child and Youth Rehabilitation and Canadian Association of Pediatric Health Centre committees as appropriate

Your Qualifications

Degree in health-related field required
Registered, or eligible for registration, with the appropriate professional body in Nova Scotia required.
Minimum five (5) years experience in a related clinical role required.
Three (3) years experience in a pediatric healthcare facility.
Three (3) years related leadership experience.
Minimum two (2) years experience working with children and youth with disabilities and their families.
Valid minimal Class 5 Nova Scotia driver's license and reliable transportation. Some travel required.
Current successful completion of the Heart and Stroke Foundation of Canada (BLS) Program within the last 12 months, or successful completion of a BLS Renewal Challenge Certificate.
Certification in First Aid.
Demonstrated knowledge of best practices in pediatric rehabilitation.
