The Lower Columbia Community Health Centre Network Society is a not-for-profit organization committed to providing holistic, accessible healthcare to all, including the vulnerable populations we serve. Located amidst the stunning natural beauty of the Kootenay region, our organization is responsible for the development and operational oversight of a new Community Health Centre in Trail, BC.
The Role
We are seeking a Social Worker (SW) for our interdisciplinary team. As a Primary Care Social Worker, you will work closely with patients and families to provide support and assistance that contributes to holistic healthcare treatment within the clinic setting. SWs assist patients, providers and the larger team with services that enable every patient to strive for optimal health and wellbeing.
As part of the interdisciplinary team, the Primary Care SW will contribute to individualized patient care plans. Contributing to the provision of team-based care, the SW supports patients and families with mental health and well-being, ensure they have the resources they want or need to optimize their health and assist them in navigating the healthcare system.
Clinical Responsibilities and SW Tasks
Conducts comprehensive psychosocial assessments of patients and families, using a variety of assessment tools to support treatment and care planning within the interdisciplinary team
Adherence to sanctioned professional standards, clinical policies, legislation and client-centered practice
Contribute to individualized care plans based on patient needs, capacity and personal goals, supporting treatment objectives that facilitate change in coping style, behaviours, attitudes and feelings
Provide counseling and support to patients and families. . . .
Takes responsibility for professional development related to Primary Care, and as required by the BCCSW to ensure comprehensive patient care
Advocate for patients' needs and rights within the healthcare system
Assist patients in navigating the healthcare system and accessing resources
Community liaison and referrals, as needed to support continuity of care for patients
Provide crisis intervention and emergency support within scope of practice & CHC patients and families about healthcare options and resources
Collaborate with community organizations to connect patients with additional services
Document patient interactions per required standards Participate in Care Conferences to review and update patient care
Maintain patient records by documenting assessments, progress notes and reports; uploading completed applications and legal documents to ensure an ongoing, accurate running record of services provided
Maintain related records, such as workload statistics, data collection and summarizing reports as required and/or requested
Assist patients to resolve financial concerns and connect them to relevant resources
Form completion on behalf of the patient, often in collaboration with the interdisciplinary team to support patient needs, e.g. disability forms, transportation, third party forms, and other forms necessary for patient functioning (better word?)
Provide bereavement counseling and support; referral to other programs, if appropriate and with patient consent
Arrange and participate in joint home visits to clients/families with other team members, as deemed necessary to support and optimize patient care
Contribute to assessment and determination of client capability, and every person's ability to make decisions related to their own care; follow legal procedures related to client incapability, respecting clients' rights and privacy
Adhere to legal and ethical guidelines regarding confidentiality and record-keeping
Stay informed regarding Social Work practice, policies, and regulations
Respond to ethical challenges in a productive manner
Qualifications and Skills
Master's Degree in Social Work (MSW preferred; BSW); must be a member in good standing, registered with the BCCSW
Minimum of two years of experience in a Social Work setting.
Ability to collaborate effectively with an interdisciplinary team on the development and implementation of processes that support team-based care
Competency in medical terminology and healthcare policies and procedures;
Ability to work effectively with all presenting patients, based on a commitment to diversity, equity and inclusion, in a setting where all are welcome, and in accordance with the Canadian Human Rights Act (1977) and the Canadian Charter of Rights and Freedoms (1982)
Cultural competency and awareness: Working knowledge of First Nations, Metis and Inuit Health, culture and traditions
Experience with Electronic Medical Records (EMRs)
A commitment to uphold the highest degree of confidentiality, with a keen awareness of practicing in a small, rural community
Knowledge of relevant laws and regulations governing Social Work practice, as well as other legislation directly pertinent to health care provisionAbility to work independently and as part of a team
Knowledge of community resources and services
Strong organizational and time management abilities
Excellent written and verbal communication skills
Valid Class 5 Driver's License
