Job Summary

We are seeking a compassionate and dedicated Registered Practical Nurse (RPN) to join our healthcare team. The ideal candidate will provide high-quality nursing care to patients across various settings, including long-term care, assisted living, and acute care environments. As an RPN, you will be responsible for assessing patient needs, administering medications, and collaborating with healthcare professionals to ensure optimal patient outcomes.

Duties

Conduct thorough patient assessments and develop individualized care plans. Administer medications and treatments as prescribed by physicians. Monitor vital signs and report any changes in patient conditions. Provide assistance with daily living activities and personal care. Collaborate with multidisciplinary teams to coordinate patient care. Maintain accurate medical documentation in compliance with HIPAA regulations. Educate patients and families on health management and disease prevention. Assist in emergency situations by performing triage and basic life support as needed. Participate in discharge planning and follow-up care coordination.

Skills

The following skills are essential for success in this role:

Proficiency with electronic medical record (EMR) systems such as Epic, Cerner, or Athenahealth. Experience in critical care, emergency medicine, or acute care settings. Knowledge of ICD-9 and ICD-10 coding for accurate medical documentation. Familiarity with long-term care, assisted living, and memory care practices. Ability to perform IV insertion, phlebotomy, catheterization, and medication administration. Strong understanding of anatomy, physiology, and medical terminology. Experience working with diverse populations including geriatrics and individuals with developmental disabilities. Skills in pain management, hospice care, and behavioral health support. Competence in conducting physical examinations and diagnostic evaluations. Excellent communication skills for effective patient education and collaboration with healthcare teams.
