Job Overview

We are seeking a dedicated and compassionate Registered Practical Nurse (RPN) to join our healthcare team. This role requires a commitment to providing high-quality nursing care to seniors with Alzheimer's and dementia. The RPN will work collaboratively with healthcare professionals to ensure the well-being and comfort of patients while adhering to best practices in medication administration and clinical procedures.

Responsibilities

Deliver comprehensive nursing care to patients in accordance with established protocols and guidelines. Administer medications and treatments as prescribed, ensuring accurate documentation in patient records. Monitor patient conditions, including vital signs, and report any changes to the healthcare team. Provide support for patients with acute care needs, dementia care, pain management, and hospice care. Assist in case management by coordinating patient services and collaborating with interdisciplinary teams. Conduct phlebotomy procedures and manage laboratory specimens as required. Utilize electronic health record system Point Click Care Educate patients and their families on Assessments and Care Plans Maintain a clean and safe environment for both patients and staff while adhering to infection control protocols.

Experience

Candidates should have:

Previous nursing home experience is highly desirable. Familiarity with acute care settings, dementia care, home care, assisted living, senior care, or hospice care is preferred. Proficiency in medical terminology and experience with medication administration is essential. Knowledge of pain management techniques and laboratory procedures will be beneficial. Strong communication skills and the ability to work effectively within a team-oriented environment are crucial for success in this role.
