Responsibilities



Deliver comprehensive nursing assessments by evaluating vital signs, conducting physical examinations, and reviewing medical histories to inform personalized care plans. Administer medications and injections accurately while adhering to all safety protocols, including IV infusions and catheterizations. Manage complex cases involving dementia care, Alzheimer's disease, memory care, and behavioral health challenges with patience and professionalism. Assist with medical procedures such as wound suturing, tube feeding, airway management, and spinal taps in accordance with aseptic techniques. Utilize electronic health record (EHR) systems like Epic or Cerner for precise documentation of patient care activities, medication administration, and medical records management. Coordinate discharge planning and collaborate with interdisciplinary teams to ensure seamless transitions of care within hospital settings or post-acute environments. Conduct triage assessments in urgent or emergency situations, providing critical support in emergency rooms or Level I trauma centers. Support infection control practices and maintain compliance with HIPAA regulations to protect patient confidentiality. Engage in health coaching and patient education initiatives tailored for seniors, children, or individuals with disabilities to promote wellness and self-care strategies. Participate in case management activities for patients with complex needs such as those requiring hospice care or palliative services. Demonstrate proficiency in medical terminology, ICD coding (ICD-9/ICD-10), CPT coding, and diagnostic evaluation processes. Provide compassionate care for patients with acute pain management needs or chronic conditions like cardiac issues or infectious diseases. Support specialized areas such as neonatal ventilator management, pediatric outpatient clinics, or home & community care programs as needed.

Requirements



Valid registration as a Practical Nurse with the appropriate licensing body in your region. Proven experience working in diverse healthcare environments including hospitals (ICU experience preferred), nursing homes, long-term care facilities, assisted living communities, or home care settings. Demonstrated expertise in critical care scenarios such as Level II trauma centers or emergency medicine environments. Familiarity with EMR/EHR systems like Epic, Cerner, Athenahealth or eClinicalWorks for accurate documentation and communication. Experience administering injections, phlebotomy (venipuncture), catheter care, and specimen collection/processing. Strong knowledge of physiology, anatomy, medical terminology, and diagnostic evaluation techniques. Ability to perform physical examinations and vital signs assessment efficiently while maintaining patient comfort and dignity. Competence in infection control practices and sterile processing procedures. Excellent communication skills to work effectively with patients of all ages--including children--and multidisciplinary teams across various healthcare settings. Knowledge of HIPAA regulations ensuring confidentiality of patient information at all times. Additional skills such as hospice or palliative care experience; working with people with developmental disabilities; dementia or Alzheimer's care; pain management; respiratory support including ventilator management; or specialized pediatric skills are highly valued.
