Summary

We are seeking a compassionate and skilled Registered Practical Nurse to join our team in providing high-quality service in long term care. Responsibilities

Provide direct patient care, including administering medications and treatments as prescribed. Conduct thorough patient assessments and develop individualized care plans. Collaborate with healthcare teams to ensure comprehensive patient management. Maintain accurate medical documentation and records for all patient interactions. Educate patients and their families about health conditions and treatment plans. Monitor patients' vital signs and report any changes in condition. Assist with IV infusions and other specialized nursing procedures as needed. Promote a safe and supportive environment for patients receiving home care.

Qualifications

Valid Registered Practical Nurse license in the state of practice. Strong knowledge of medical terminology and documentation practices. Ability to perform IV infusions and other nursing procedures competently. Excellent communication skills with a focus on patient education. Experience with electronic medical records (EMR) systems is preferred. Familiarity with critical care or long-term care settings is a plus. Compassionate approach to nursing with a commitment to quality patient outcomes.
