We are seeking a dedicated Registered Practical Nurse to join our healthcare team. The Nurse is required to provide and assist with the implementation of resident care plans based upon pre-established standards and assist in planning of personal care to each resident under the direction and supervision of the Director of Wellness. The ideal candidate will provide compassionate and high-quality care to patients in a community setting.
Functions:
Assists the Director of Wellness with planning of resident care plans.
Complies with all relevant Provincial and Federal Acts and Regulations, e.g., RHRA, Resident Rights, Public Health, Employment Standards Act, etc.
Responsible for admissions and discharges of residents.
Is responsible for control and safekeeping of drugs and narcotics.
Meets with residents' families to resolve problems/concerns/complaints and review suggestions on a need be basis.
Maintains contact with residents to ensure a high quality of care.
Ensures the nutritional needs of the residents are met in collaboration with the Food Services Manager
Reviews each resident's profile and care plan to ensure that problems are dealt with appropriately and that individual needs are met.
Ensures the availability and maintenance of nursing department supplies and equipment.
Ensures an Infection Control Program is implemented and monitors and coordinates the overall program on an ongoing basis, with the Director of Wellness
Attends resident meetings as requested to discuss concerns/complaints/ suggestions.
Promotes interaction with other local health care facilities and educational institutions.
Establishes and maintains good working relationships with all staff, resident families, the community, the physicians, and other professionals.
Maintains a necessary degree of nursing knowledge and skills through seminars and courses.
Performs personal care and selected nursing measures to promote the comfort and safety of the residents under the supervision of a Registered Healthcare Professional such as Director of Wellness.
Attends the care conferences as requested by the Director of Care/designate to assist in the formulation of a comprehensive plan of care for each resident.
Observes and promptly reports changes in a resident's physical and emotional condition.
Maintains accurate documentation, according to established policies, reflecting each resident's condition, care, and treatments.
Deals tactfully and courteously with residents, residents' families, and visitors.
Attends staff meetings as required - to know changes in the facility that have taken place and update knowledge in better caring for the residents.
Reports all accidents and incidents promptly and accurately.
Knowledge of home care services, both private pay and government funded.
Performs other duties as requested by the Executive Director and/or Senior Management.
Qualifications:
1. Current registration as a Registered Nurse/Registered Practical Nurse with the College of Nurses of Ontario.
2. Possession of certificate of basic life support at basic rescuer level (minimum).
3. Must be a competent person as defined by the Occupational Health and Safety Act Competency Criteria
Has knowledge of any potential or actual danger to health or safety in the workplace.
Experience in geriatric, rehabilitation or long-term care nursing is desirable.
Knowledge of the nursing process.
Post-graduate study in geriatrics desirable.
Post-graduate study in nursing administration desirable.
Possession of good leadership, communication, and diplomatic relations skills.
Possession of skill in planning, and coordination of a comprehensive nursing care program.
Reports to:
Reports directly to the Director of Wellness.
Working Condition:
Must be able to stand for long periods of time and walk long distances during work period.
Lifting alone, maximum 40 lbs. (5-25 lbs. usual) as an essential job function.
Lifting with assistance, maximum 160 lbs. (100-200 lbs. usual) as an essential job function.
Pushing/pulling, using wheeled equipment such as carts and trolleys, maximum 100 lbs. (20-50 lbs. usual) as an essential job function.
Stretching/reaching on a regular frequency throughout the shift as an essential job function.
Walking, with possible some upstairs walking, up to 3 hours maximum, (1-2 hours usual) as an essential job function.
Standing, for lengths of time, up to 3 hours maximum (1-2 hours usual) as an essential job function.
Bending/stooping on a regular frequency throughout the shift, as an essential job function.
Crouching/kneeling on an irregular frequency throughout the shift, as a non-essential job function.
Carries/transports objects of various sizes, up to a maximum of 25 lbs. (5-15 lbs. usual) as an essential job function.
Must be able to perform turning, stooping, bending, stretching, and lifting to assist residents in care requirements and other related duties.
Must be able to move and lift equipment and supplies of various sizes and weights.
Must have visual and hearing acuity to recognize the need of assistance required by residents.
Will be required to work every other weekend.

2. Works safely to reduce the risk of injury to self, co-workers, and residents.
3. Is alert to and promptly reports all actual or potentially hazardous situations to immediate supervisor. Does not operate or use faulty equipment.
4. Wear personal protective equipment (or clothing) as required by task, M.S.D.S. or facility policy.
5. Promptly reports personal injury to supervisor and seeks first aid as needed.
6. Participates in fire safety demonstrations and fire drills and knows the facility fire and disaster plan.
