Job Description

Partners Community Health (PCH) is a new not-for-profit organization, focused on bringing healthcare services together around the needs of people living in Mississauga and West Toronto. PCH currently operates two new state-of the-art LTC homes and community hub in West Mississauga. The new homes have a combined 632-beds and be part of PCH's larger strategy that introduce innovative and inclusive programs and services and new models of care delivery. PCH is committed to building partnerships and connections that put people first, as well as a learning and leading healthcare community that provides best in class service across the aging continuum.



Key Responsibilities:



As a Registered Practical Nurse (RPN), you will play a key role in supporting seniors who access the Integrated Seniors Hub by delivering compassionate, person-centered, and community-driven care.



Community Driven:



Establish and maintain respectful, positive relationships with clients, families, caregivers, team members, volunteers, and community partners. Act as a resource and advocate for seniors, connecting them with services and programs that enhance their health and well-being.

Caring Service:



Collaborate closely with members of the multidisciplinary care team to develop and implement individualized care plans. Administer medications and treatments in accordance with CNO standards, including medication management, narcotic control, and safe disposal of discontinued or expired medications. Perform wound care, dressing changes, immunizations, and other nursing interventions as required. Support chronic disease management, health education, and wellness promotion for seniors and their caregivers. Uphold the Patient's Bill of Rights and the Hub's commitment to dignity, respect, and inclusion. Support client rights and autonomy while fostering a safe and supportive environment. Perform other duties as assigned to meet client needs and organizational goals.

Professionalism:



Be accountable to nursing leadership for delivering safe, evidence-based nursing care and clinical interventions. Adhere to the College of Nurses of Ontario's Guidelines for Ethical Behavior in Nursing and relevant legislation. Contribute to a positive, collaborative work environment that values teamwork and mutual respect.

Innovative Approaches:



Contribute ideas and feedback to enhance care models, programs, and procedures. Facilitate team collaboration and embrace change to improve client outcomes. Apply evidence-informed practices to problem-solving and decision-making. Seek ongoing professional development opportunities to maintain competencies and support continuous quality improvement.

Qualifications:



Post-graduate studies certification in Geriatrics and Nursing Administration is an asset One year of work experience in a seniors setting with knowledge and interest in gerontology Members R.P.N.A.O./R.N.A.O./Gerontological Nursing Association preferred Applicants must successfully pass the prescribed Vulnerable Sector Check

Job Requirements:



Member of CNO with Active RPN License Valid First Aid & CPR Certification Must present a clear Vulnerable Sector Check (completed in the last 6 months) Completed 2-Step TB Test Must be available to work weekends and statutory holidays
