Wyndham House is an organization that provides young people 16 to 25 with a spectrum of supports and services needed to overcome challenging times and difficult problems related to housing and homelessness. Wyndham House programs include two Permanent Supportive Housing Programs, an Emergency Shelter, a Housing Stability Outreach Team, an Integrated Youth Service Hub and a Care and Treatment Day Program. Wyndham House is a fast-paced and growing organization that works to prevent, reduce, and end youth homelessness, as part of the Built for Zero change effort, which is working to end chronic homelessness in Canada. Wyndham House provides high-quality case management, housing, and clinical supports to youth with a variety of needs and complexities.
Position Summary:
Reporting to the Manager of Health & Education, the Registered Practical Nurse will provide medical assessments and treatment care to the youth homeless population within Wyndham House. As a Registered Practical Nurse, you will be an integral part of a collaborative and communicative team, which will all focus on delivering medical and outreach services to our clients. In collaboration with the Hart Hub team, the Registered Practical Nurse will conduct assessments and provide treatment and rehabilitation services as well as performing professional nursing care to the youth at Wyndham House.
Responsibilities and Duties:
Provide medical care including medication oversight and support, wound care, crisis intervention and psychoeducation
Provide professional nursing care to youth who are struggling with mental health illnesses, addictions, substance abuse and other disorders
Contribute to all patient needs in accordance with CNO standards and RNAO Best Practice guidelines
Administer medications and treatments, following assessments through physical examinations and interviews to review youth medical and mental health history
Ensuring safety and security of health care services such as security of pharmaceuticals and medical supplies
Reviewing, observing, recording, and reporting youth's health care treatment, conditions, and behavior with reports and documents
Act as a coach, nurse, mentor or as a resource to the youth and other staff members when appropriate
Skills and Qualifications:
Current registration as a Registered Practical Nurse with the College of Nurses of Ontario, in good standing
Undergraduate degree from an accredited University or College, preferably in Bachelor of Science
Minimum of 2 years of experience as a Registered Practical Nurse or equivalent
Broad range of experience working and treating patients, especially within the youth sector, who struggle with additions, mental health illnesses, physical health issues such as substance abuse
Experience providing treatment to patients
Strong communication and interpersonal skills
Working knowledge of the Mental Health Act and community mental health resources
Knowledge and skills on crisis management and crisis intervention
Ability to work under pressure
Computer skills: MS Word, MS Outlook, MS Excel

Requirements:
Registration with the College of Nurses of Ontario (C.N.O) in Good Standing
Emergency First Aid/CPR
