The Registered Practical Nurse reports to the Director of Care and is expected to coordinate and oversee all aspects of resident-focused delivery of care. Provide direct supervision, leadership and education to other members of the healthcare team. Acts as a resource advocate for residents, families, and staff. Promotes the values, goals, and philosophy of the Home. Responsibilities: \xe2\x97\x8f Development and modification of the plans of care in response to residents\xe2\x80\x99 individualized health care needs and change in health care status \xe2\x97\x8f Liaises with other health care personnel, agencies and families \xe2\x97\x8f Maintains good relationships with Medical/Support/Home staff, residents, families and general public \xe2\x97\x8f Supervises all activities on the unit, takes charge as required \xe2\x97\x8f Monitors performance and completes performance evaluation for the PSW\xe2\x80\x99s and RPN\xe2\x80\x99s \xe2\x97\x8f Directs, assigns and evaluates the work of the PSW\xe2\x80\x99s \xe2\x97\x8f May be in charge of Home as required. Utilizes leadership skills to identify problems and problem-solving on the unit \xe2\x97\x8f Monitors that care is delivered is consistent with the resident Bill of Rights. \xe2\x97\x8f Required to understand the nature and meaning of quality indicators used by the Home and to understand which indicators are publicly reported. \xe2\x97\x8f Required to participate in the improvement of the indicators and achieve satisfactory results. \xe2\x97\x8f Orders and ensure nursing supplies and equipment are properly and efficiently used \xe2\x97\x8f Actively listens to residents and family concerns, take appropriate actions to resolve the issue in accordance with policies of the Home \xe2\x97\x8f Conducts unit rounds at the beginning and end of shift. \xe2\x97\x8f Processes Physician orders and administers medications and treatments in compliance with the policies of Pharmacy provider, the Home and the Standard of the College of Nurses. Documents and reports all errors and omissions .\xe2\x97\x8f Maintains accurate and complete records of nursing observations and care in compliance with the legislative requirements and the policies of the Home \xe2\x97\x8f Assesses changes in health status and reports changes to Substitute Decision Maker (SDM) or POA, and interdisciplinary team \xe2\x97\x8f Completes health care assessments (RAI-MDS and other assessments), develops and updates plan of care for each resident in collaboration with the resident and/or SDM and the interdisciplinary team. \xe2\x97\x8f Demonstrates understanding of the Ministry of Long-term Care mandatory reporting and critical incidents and follow steps outlined in the policies of the Home. \xe2\x97\x8f Other duties as assigned and or indicated in Job Task Inventory Qualifications: The qualifications needed to join our family are as follows: \xe2\x97\x8f Current Certificate of Competence with the College of Nurses of Ontario as a Registered Nurse or a Registered Nurse registered with a regulatory authority in a Canadian jurisdiction, other than Ontario, and holds in that jurisdiction the equivalent of a certificate of registration authorizing independent practice in Ontario as of right exemption Bill 60 \xe2\x97\x8f Basic CPR \xe2\x97\x8f Experience in long-term care setting an asset \xe2\x97\x8f Ability to work with and relate to interdisciplinary team members \xe2\x97\x8f Well-developed written and oral communication skills \xe2\x97\x8f Knowledge of RAI-MDS \xe2\x80\x93 AIS certified, if not must be completed successfully within one (1) year of being hired. \xe2\x97\x8f To show evidence of being focused on people\xe2\x80\x99s feelings when interacting and knowing how to turn a task into positive social interaction. \xe2\x97\x8f To undertake and meet all legal requirements within a context of promoting individuals\xe2\x80\x99 rights and measuring/reducing elements of risk. \xe2\x97\x8f Must provide a complete and current (within six months) Vulnerable Sector Check (including a Criminal Background Check) or be willing to obtain one \xe2\x97\x8f As a condition of employment, candidates are required to submit documented results of TB testing (within the last six months or within 14 days upon hire) or a chest X-ray (within the last year), as per Public Health requirements \xe2\x97\x8f Two supervisory references required What do we offer you? \xe2\x97\x8f Competitive wages \xe2\x97\x8f Employee benefits \xe2\x97\x8f Employee and Family Assistance Program \xe2\x97\x8f Support for personal and professional growth We look forward to speaking with you and Making Every Moment Matter\xe2\x84\xa2.
