Job Description

The Beamsville Medical Centre Family Health Team (BMCFHT) is looking for a Registered Practical Nurse (RPN) to assist at a busy family physician office. The RPN collaborates with Family Physicians, Nurse Practitioners, Registered Nurses, and our interdisciplinary team, and staff to provide comprehensive nursing care and clinical support within their full scope of practice as defined by the College of Nurses of Ontario (CNO) Standards of Practice. You will work independently, as well as in consultation and collaboration with the Family Health Team staff. The RPN supports the delivery of high-quality, person-centered care across a variety of clinical and preventative services. The position will be a one year contract with the potential after one year of becoming a permanent position.

Education, Qualification and Skills

RPN with current registration with the College of Nurses of Ontario

Evidence of strong clinical skill sets

Previous experience navigating patients through the health care system

Excellent communication skills (both oral and written) and organizational skills

Comfort and ease using technology (computers, Internet)

Previous experience with electronic medical records (EMR) TELUS an asset

Responsibilities

Conduct patient interviews to gather information about the presenting health concerns Perform focused assessments based on patient need, using appropriate tools and techniques Document relevant health history, medications (including OTC and alternative), allergies, and intolerances in the EMR Apply clinical reasoning to identify and communicate potential health concerns Utilize best practice guidelines for monitoring and supporting patients' physical and emotional health Provide in-person and telephone-based triage and support, following established protocols Assist with routine procedures, including immunization administration, wound care, and vital signs monitoring Participate in preventative health activities, such as flu clinics Maintain clinical documentation in accordance with organizational and professional standards Other duties as assigned to support the effective operation of the clinic and BMCFHT programs and services Participate in regular BMCFHT meetings, trainings, and programs as required

Pay: $27.00-$31.00 per hour

Expected hours: 24 - 32 per week
