The Allied Health Services (AHS) team, under Professional Practice consists of Regulated Healthcare Professional including: Nursing, Physiotherapy, Occupational Therapy, Speech Language Pathology, Social Work, Registered Dietitians, as well as unregulated Rehabilitation Assistants. Each discipline is focused on driving evidence informed practice to promote high quality patient and family centered care.

The team has a professional practice leader (PPL) model for each regulated allied health discipline which promotes and enhances the interprofessional team. Each discipline has their own PPL with dedicated time to enhance their communities of practice, represent their respective disciplines at committees, create policies & procedures and lead corporate level initiatives.

We are currently seeking a temporary part time Ambulatory Day Rehabilitation Program Patient Care Navigator (November 2023 - September 2024) to support our adult ambulatory Day Rehabilitation Program. This program provides outpatient services to patients with subacute stroke or lower extremity amputations. As part of an interprofessional care team, the successful candidate will:

  • Act as a primary contact and liaison between patients, families and Ambulatory Day Program Services and/or community resources.
  • Build Effective working relationships and engage patients and families to discuss care options while advocating for the individual needs of patients and supporting patient choice in health care decision-making
  • Assist in the provision of health care teaching including individual and/or group based activities
  • Provide nursing-specific interventions to support patient care needs
  • Employ a patient-centered approach to identifying factors that might present barriers to care
  • Ensure patients and families have information and resources needed to make informed decisions and be active participants across the care continuum
  • Connect patients to healthcare teams and/or resources as required
  • Support patients to take ownership of their care throughout their stroke or amputation care journey.
As a qualified candidate you will have the following job requirements:

Education:
  • Registered Practical Nursing Diploma or equivalent to practice at full scope - mandatory
  • Current Certificate of Registration form the College of Nurses of Ontario -mandatory
  • Completion of recognized Medication Administration Course.
  • Current BCLS required.
Experience:
  • Minimum 5 years\' experience working with patients with subacute neurologic and vascular diagnoses; specifically patients on the stroke care continuum as well as patients with lower extremity amputation.
  • Direct experience in the provision of healthcare teaching and recommendations for patients along the care continuum post stroke or amputation.
  • Solid knowledge and application of stroke best practices and care pathways.
  • Experience and familiarity using excel and electronic documentation software.
  • Experience in the collection, tracking and submission of data to support patient care initiatives.

  • Possess excellent communication skills and the demonstrated ability to communicate effectively with patients, families and colleagues.
  • Strong ability to build and maintain relationships as well as sensitivity and responsiveness to diverse patient populations.
  • Ability to exercise discretion and confidentiality at all times.
  • Proven organizational and time management skills and aptitude employing virtual, electronic and personal communication platforms.
  • Superior problem solving skills and the ability to negotiate the resolution of conflict.

Please be reminded that as a Regional Health Centre, in order to provide the necessary patient care, our operations run 24/7.
