Position Summary
:
The Registered Practical Nurse is a regulated health professional who is responsible and accountable for the quality of patient care based on the Professional Standards for Registered Nurses and Registered Practical Nurses in Ontario. The RPN independently cares for patients whose outcomes are predictable and whose conditions are less complex. These patients present to hospital with well-defined, established care needs, coping mechanisms, and support systems. The RPN provides holistic patient care, educates patients about various health conditions, and provides emotional support to patients and their family members. The RPN collaborates with the Registered Nurse in patient situations that are beyond the RPN scope of practice.
Responsibilities:
Establishes and maintains a therapeutic relationship with patients and their families.
Develops a comprehensive, individualized plan of care in collaboration with the patient, their family, and the interdisciplinary team.
Implements patient care interventions within professional scope of practice and as ordered by the most responsible physician.
Participates in interdisciplinary team rounds, patient care conferences, and discharge planning meetings.
Provides initial and ongoing assessment and interpretation of patient's health status within the framework of professional nursing practice, and in accordance with the principles of patient and family centered care.
Assessment:
Assesses and interprets information regarding the patient's culture and values system, healthcare goals, and their desire to participate in their care.
Assesses patient goals for care I discharge and ensures supports are in place to facilitate the achievement of these goals.
Ensures the implementation, coordination and ongoing evaluation of the patient care plan including the documentation of patient's responses to interventions and achievement of expected outcomes.
Collaborates with the RN when patient complexity exceeds professional scope of practice.
Documents complete, accurate and relevant information.
Collaborates with RN when patient acuity and/or complexity exceeds RPN scope of practice.
Evaluation and Professional Practice:
Ensures the implementation, coordination and ongoing evaluation of the patient care plan including the documentation of patient's responses to interventions and achievement of expected outcomes.
Documents complete, accurate and relevant information.
Collaborates with RN when patient acuity and/or complexity exceeds RPN scope of practice.
Supports the orientation of new staff members and students to the service or unit by sharing his/her nursing knowledge and contributing to a supportive learning environment.
Advocacy and Safety:
Advocates for and supports the principles of patient and family centered care.
Assists patients and families to become aware of issues relevant to their health care needs and provides information on resources and services within the health care system.
Practices infection control standards and precautions to promote safety.
Participates in and supports activities related to professional practice, quality improvement and patient safety.
Position Requirements:
Diploma of registered practical nursing or equivalent from an accredited institute
Registered Practical Nurse in good standing with the College of Nurses of Ontario (CNO)
Current certification and ongoing maintenance of BLS Certification
Completion of a cardiac interpretation course(s) through an accredited institute (within 3 months of hire)
Completion of 12 lead interpretation course through an accredited institute (within 12 months of hire)
Completion of Coronary Care Certificate from an accredited institute (within 3 years of hire)
Act as a preceptor/mentor for learners including but not limited to students, new hires, clinical externs.
Ability to work effectively as a member of an interdisciplinary team.
Proven ability to plan and organize with good critical thinking and problem-solving skills.
Excellent verbal and written communication skills as well as strong public relation/interpersonal skills
Evidence of ongoing professional development/continuing education and participation in quality assurance activities
Positive work record and good attendance record; ability to meet the physical demands of the position.
Preferred Requirements:
Have worked in a similar unit in an acute care setting within the last three (3) years.
Have worked with a similar patient population within the last three (3) years.
Experience in using electronic medical record systems (Cerner).
Completion of a physical assessment course through and accredited institute.
Knowledge, skill, and experience in: management of cardiac diagnosis' (e.g. heart failure, acute coronary syndrome), cardiac medications, anticoagulation therapy.
