Carefirst Seniors & Community Services Association
is a non-profit charitable community services agency established since 1976. Our spectrum of services has grown from the delivery of Chinese meals-on-wheels service to a full range of community support services, covering the whole Greater Toronto Area.
With a staff of 500 members (including full-time and part-time staff) and the help of a pool of 850 active volunteers, we serve over 15,000 clients a year, including seniors, adults, youth with physical, social, emotional and health challenges. 3,000 of whom are "home-bound" and frail.
We are currently seeking a
Registered Practical Nurse
to join our Home Dialysis Assistance Program (HDAP) team.

Location:
Scarborough & west end Toronto (Bloor St. W / Dupont; Eglinton W / Royal York; St. Clair W / Caledonia; Jane / Lawrence W)
Responsibilities:
Provide assistance with peritoneal dialysis to clients and caregivers in the community setting (including patients' homes)
Encourage self-management by providing tools and resources to enhance clients' and caregivers' knowledge, skills and motivation to engage in shared care in PD
Collaborate with hospital partners and HDAP Team Lead to ensure full coverage of client needs
Perform nursing duties in adherence to agency policies and scope of practice as per regulatory colleges
Monitor, assess, communicate and document patient conditions and changes following best practice in order to provide appropriate and timely interventions to reduce risks and achieve optimum health results
Adhere to health and safety and other agency policies
Perform any other HDAP-related tasks as assigned
Qualifications:
Current registration in good standing as Registered Practical Nurse with the College of Nurses of Ontario
Minimum of 3 years of nursing experience in peritoneal dialysis and/or hemodialysis is an asset
Knowledge of adult education teaching principles, or strategies is an asset
Knowledge of chronic kidney disease, pathophysiology and management
Excellent written and verbal communication skills in English. It is an asset to speak other languages that are used by the communities we often serve
Valid Ontario driver's license, and a reliable vehicle required for home visits is a must
Excellent customer service with strong interpersonal, communication, and problem solving skills
Capable to work independently with minimum direct supervision
Proficient computer skills in Microsoft Office and EMR (Telus Practice Solutions)
Flexible and adaptable to change in work assignments and schedules (shift work, overnight, weekends, and statutory holidays)
A valid vulnerable sector police record check completed within one year
