Position Summary:



Provide hemodialysis treatments for chronic kidney disease patients in an outpatient clinic setting Collaborate with the multi-disciplinary team to develop, implement, and monitor effectiveness of individualized plans of care Responsible and accountable for decisions and actions in the provision and management of Nursing care for patients in a culture of "care partnership model" Responsible for the provision of nursing care, teaching and coaching to clients and their families Responsible and accountable for the provision of nursing care in accordance with the standards of nursing practice and THP Best Practice Protocols This position will transfer to the Speakman and Watline satellite sites within 6-12 months of hire

Qualifications:

Candidate will be a diploma graduate of a recognized college or school of nursing or equivalent (i.e., certificate RPN with added courses in medication administration, phlebotomy, and health assessment) Current certification or eligibility for certification of competence from the College of Nurses of Ontario is required Current BCLS Minimum of two years recent medical/surgical experience in an acute care setting or 1 year experience in Hemodialysis or Pertioneal Dialysis required. Completion of Nephrology Nursing Certification courses is preferred. Demonstrates knowledge of care for the patient with end stage renal disease Recent experience with the renal patient population preferred Demonstrates knowledge of and experience with the following: wound, skin and foot care; the diabetic patient; venipuncture; routine practices of infection control; normal and abnormal blood values Demonstrates technical ability Previous experience in patient education programs Demonstrates excellence in communication skills and team work Demonstrates strong attention to patient and family-centred care and establishing therapeutic relationships Evidence of ongoing professional development preferred Assignments will include: Renal Care Centre satellite unit, CVH and the Mississauga Site A positive work and attendance record is essential
