CORE RESPONSIBILITIES OF THE POSITION
The Physiotherapist will provide assessments, education and counselling as well as physiotherapy activities for seniors living with Chronic Diseases and/or physical disabilities. As a key member of the inter-professional Chronic Disease Management team, the Physiotherapist will:
Implement appropriate therapeutic strategies by working with the inter-professional team using a client centered approach
Assist in maintaining therapeutic areas, resources, materials and equipment
Assessing client fitness status and gain an understanding of the client's goals, psychological outlook, cultural background and clinical profile
Establish therapeutic relationships and develop fitness plans incorporating clients' health goals, culture and income
Provide written and verbal instructions to clients, translating scientific information about exercises into practical fitness advice for people in a fitness program
Educate and advise a wide range of clients with chronic disease-related disorders on the practical ways in which they can improve their health by adopting healthier fitness habits
Contribute to chart documentation as per college and weCHC standards; participate in chart reviews and case conferences
Communicate client progress to referring/non-referring Physicians, Respirologists, Neurologists, or Primary Healthcare providers as needed
Ensure ongoing fitness goals by monitoring client progress on a regular basis and working within the client's circle of care to ensure positive outcomes for the client.
Participate in identifying needs within the community and developing programs which address those needs as appropriate to the weCHC strategic plan
Collaborating with community partners to support population health from a fitness perspective.
Support system navigation by making referrals both internal and external to other community services as appropriate
Perform other duties that support the mission and vision of the organization and/or as assigned based on position responsibilities and/or site requirements.
SKILLS, KNOWLEDGE & EXPERIENCE REQUIRED
3-5 years of recent experience in Physiotherapy
Experience working with the senior population and adults with chronic diagnoses, with preference to those who have worked in a community health setting
Proficiency in functional mobility and fitness assessments. Strong knowledge and experience with functional exercise for adults and seniors living with chronic conditions
Thorough knowledge and understanding of community resources specific to Windsor/Essex County
Trained in proper transfer techniques, to ensure clients are transferring safely
ADP (Assistive Devices Program) authorization as an asset
Excellent communication and presentation skills with a demonstrated ability to undertake fitness counselling (individual and group)
Demonstrated ability to work independently and as a team member within an evolving role in an inter-professional environment
Strong commitment to health promotion, community development and adult education
Ability to work in a variety of social and physical settings
Experience in program development, implementation, monitoring and evaluation
Excellent follow-up and follow through skills with ability to identify and resolve problems in a timely manner
Clean driver's abstract, current, valid Class "G" Driver's License and reliable transportation to support travel across Windsor-Essex County and the province, as required
Bilingual (English/French) language skills an asset
Education and certificate requirementS
Degree in Physiotherapy or Master of Science in Physiotherapy from a recognized University
Member in good standing and current registration with the College of Physiotherapists of Ontario
Completion of a Vulnerable Sector Check prior to commencing employment.
