About the Opportunity
Reporting to the Pharmacy Manager/Operations Manager, the Registered Pharmacy Technician (Pharmacy Practice Assistant) participates in the provision of safe, competent, patient or family centered ethical care, ensuring the goals and needs of the patient are prioritized and individualized; prepares, packages and dispenses medication under the direct supervision of a pharmacist. Responsibilities include compounding of sterile and non-sterile dosage forms, dispensing drugs to inpatients and outpatients in unit dose, physician order entry, working with automation, Inventory control, medication reconciliation, and CIVA systems. Preparation of chemotherapy doses, distributing narcotics and participating in quality programs and patient safety initiatives.
Responsibilities include obtaining a best possible medication history (BPMH) for patients being admitted from emergency department to an inpatient hospital unit or non-admitted patients; for patients seen in Surgery Preassessment clinic and for inpatient units. Conducting patient medication history interviews with patient and family, collecting patient information from the Drug Information System (or alternate source if not available), documenting BPMH in patient chart to be used to create admission medication orders, reconciling medication orders completed by another health care provider, communicating to staff and physicians regarding medication reconciliation, problem solving with the patient care team and pharmacy staff, medication reconciliation at all transitions of care. Medication reconciliation RPhT (PPA) acts as a resource to staff and physicians, directs team members to ask for BPMH completion, confirms accuracy of patient medication information with community care providers or patient\'s medical record.
