Job Description
Carlington Community Health Centre is seeking experienced and compassionate Registered Nurses (RNs) and Nurse Practitioners (NPs) to join our dynamic, interdisciplinary team on a casual basis. This is a rewarding opportunity to contribute to high-quality, client-centred care in a community-based setting focused on health equity and social determinants of health.
Responsibilities include
Provide high-quality, client-centered nursing care to individuals and families from diverse and marginalized communities. Conduct clinical assessments, deliver treatments, and manage follow-up care in a primary care setting. Support chronic disease management, health promotion, and illness prevention through education and clinical interventions. Collaborate with physicians, nurse practitioners, and allied health professionals to deliver coordinated, holistic care. * Maintain accurate documentation and uphold professional and organizational standards of care.
